Salary: £32,500 to £35,000 per annum dependant on experience
Hours: Full Time (37.5 hours)
Location: Based in our beautiful offices in Henley-on-Thames, with the opportunity for hybrid working (three days in the office and two days working remotely, following a successful induction and subject to operational requirements).

Purpose of the Role

The Care Coordinator/ Integrated Care Lead plays a pivotal role in the day-to-day operational delivery of Care Bridge Specialists' services. Working closely with the Integrated Care Team, the Care Coordinator is responsible for coordinating complex care packages, supporting our frontline Support Practitioners, ensuring staffing continuity and maintaining high standards of quality, compliance and person-centred care.

This is a varied role that requires excellent organisation, problem-solving and communication skills. The successful candidate will be proactive, calm under pressure and able to manage multiple priorities whilst always putting the individual receiving support at the centre of everything they do.

Key Responsibilities

Care Coordination

Coordinate the day-to-day delivery of multiple community care and support packages.

Produce, manage and amend rotas to ensure safe staffing levels and continuity of care.

Arrange emergency cover and respond to short notice staffing changes.

Support the mobilisation of new care packages, ensuring all operational arrangements are in place.

Liaise with families, commissioners, clinicians and multidisciplinary teams regarding day-to-day operational matters.

Monitor package hours and staffing requirements, escalating concerns where appropriate.

Workforce Coordination

Support Practitioners with day-to-day operational queries.

Complete wellbeing and welfare check-ins with Support Practitioners.

Coordinate induction shifts, shadowing and competency sign-offs.

Monitor attendance, sickness, annual leave and availability.

Help build small, consistent teams around each individual.

Compliance & HR Administration

Maintain accurate Support Practitioner personnel files.

Monitor safer recruitment documentation and employment records.

Track mandatory training, competency assessments and professional renewals.

Ensure DBS certificates, Right to Work documentation and other compliance records remain up to date.

Update training matrices and workforce trackers.

Systems & Administration

Maintain accurate records across company systems including:

- Nourish Care Notes

- Capsule CRM

- QCS

- Microsoft 365

- RotaCloud

Upload and maintain care documentation.

Ensure records are accurate, current and audit ready.

Quality & Governance

Support the Integrated Care Team with audits and quality assurance.

Monitor incidents, safeguarding concerns and complaints, ensuring appropriate escalation.

Ensure documentation is completed to a high standard.

Promote compliance with CQC Fundamental Standards and company policies.

Communication

Be a key point of contact for Support Practitioners, families and professionals.

Build positive working relationships with Local Authorities, NHS Trusts and Integrated Care Boards.

Support MDT meetings where appropriate.

Communicate professionally, sensitively and promptly.

What We're Looking For:

Previous experience coordinating domiciliary care, supported living or complex community services.
Excellent organisational skills with the ability to manage competing priorities.
Experience producing and managing staff rotas.
Knowledge of CQC regulations and safer recruitment.
Confident using Microsoft Office and digital care systems.
Excellent communication and relationship-building skills.
A calm, positive and solutions-focused approach.

Desirable:

Experience supporting people with Learning Disabilities, Autism and Mental Health needs.
Experience of coordinating hospital discharge, transition or crisis support packages.
Knowledge of Positive Behaviour Support (PBS) and trauma-informed approaches.
Experience using Nourish, QCS, Capsule or similar care management systems.
