Job Overview

We are seeking a dedicated and compassionate Home Care Worker to join our team and make a meaningful difference in the lives of those in need of support at home. In this vital role, you will provide personalized care and assistance to clients, helping them maintain independence, comfort, and dignity. Duties

Assist clients with daily activities such as bathing, dressing, meal preparation, and mobility support
Develop and follow personalized care plans in collaboration with healthcare professionals and family members
Supervise and support clients to promote their independence while ensuring safety at all times
Drive clients to appointments, errands, or social activities using your own vehicle or company vehicle
Maintain accurate records of care provided, including progress notes and any changes in condition
Communicate effectively with clients, families, and healthcare teams to ensure seamless care delivery
Monitor client well-being regularly, reporting any concerns or incidents promptly

Qualifications

Valid driving license with reliable access to a vehicle for client transportation
Previous experience working in care homes or providing senior care is highly desirable
Supervising experience or leadership skills to oversee client activities and assist other team members when needed
Knowledge of creating and implementing care plans tailored to individual needs
Excellent communication skills for building trusting relationships with clients and their families
Basic IT skills for maintaining accurate documentation and updating electronic records
Compassionate attitude with a strong sense of responsibility and professionalism
