Clinical Team Leader (Clinical Lead)

Location: Heatherdale Nursing Home, Hempstead, Gillingham, Kent

Hours: Monday to Friday, 7:00am – 2:30pm (35 hours per week)

Join Our Team

Heatherdale Nursing Home is looking for an experienced, compassionate and motivated Clinical Team Leader to join our dedicated nursing team.

We provide high-quality nursing, residential, respite, convalescence and palliative care, placing our residents’ wellbeing, dignity and independence at the heart of everything we do. This is an excellent opportunity for a Registered Nurse who enjoys leading by example, supporting colleagues and driving high standards of clinical care.

Working alongside the Home Manager, Deputy Manager, Head of Care and Compliance Team, you will play an important role in ensuring our residents receive safe, effective and person-centred care every day.

The Role

As Clinical Team Leader, you will provide visible clinical leadership across the home, supporting the nursing and care teams while promoting best practice and ensuring compliance with CQC standards.

This is a predominantly supernumerary leadership position, with a strong focus on quality improvement, staff development and maintaining excellent clinical standards.

Key Responsibilities

Clinical Leadership

Complete daily quality walk rounds across the home.
Monitor residents’ wellbeing, staff practice and environmental standards.
Provide clinical guidance and support to nurses and care staff.
Promote a positive, professional and person-centred culture.

Care Planning & Documentation

Ensure care plans remain accurate, personalised and up to date.
Lead the completion of care plans for new admissions within 72 hours.
Support staff with PRN protocols, risk assessments and clinical documentation.
Promote best practice in Mental Capacity Act (MCA) and DoLS documentation.

Quality & Compliance

Support the Home Manager with quality assurance and clinical audits.
Monitor compliance with CQC Fundamental Standards.
Help implement and evidence improvements identified through audits.
Contribute to continuous quality improvement projects.

Staff Development

Deliver group supervision sessions and provide ongoing coaching.
Assess and monitor Moving & Handling competencies.
Support competency assessments and staff development programmes.
Encourage teamwork, accountability and professional growth.

Is passionate about delivering outstanding person-centred care.
