Duties:

Provide person-centred care and support to individuals with a range of needs, supporting them to live independently in their own homes and communities.
Assist with daily living activities, including meal preparation, personal care (such as washing, dressing, and hygiene support), and medication support in line with training, company policies, and individual care plans.
Implement Positive Behaviour Support (PBS) strategies to promote positive behaviour and ensure the safety and wellbeing of individuals.
Observe, monitor and report changes in an individual’s health or wellbeing to the appropriate healthcare professional.
Collaborate with case managers, healthcare professionals, and families to support the development and delivery of individual care plans.
Maintain accurate care records using our digital care management system, ensuring all support is clearly documented.
Safeguard and promote the welfare of individuals by recognising and reporting any safeguarding concerns in line with company procedures.
Promote people’s rights, independence, dignity, choice and wellbeing at all times.
Maintain confidentiality in accordance with UK GDPR, the Data Protection Act 2018, and company confidentiality policies.
Support individuals to attend appointments, outings, and community activities.

Qualifications:

At least one year’s experience in a care or support role is required.
Full induction and ongoing training will be provided.
Understanding of care principles and commitment to person-centred support.
Ability to communicate effectively with individuals, families, and professionals.
Strong organisational skills and attention to detail.
Ability to support individuals with behaviours that may challenge, using Positive Behaviour Support (PBS) approaches.
A valid UK driving licence is preferred/required depending on service needs.

Shift Patterns:

Typical shifts may include:

Morning shifts
Evening shifts
Weekend shifts
Bank holidays (on a rota basis)

Additional Requirements:

Applicants must have the legal right to work in the UK
All successful applicants will be subject to an Enhanced DBS check
Satisfactory references are required prior to employment
