We're looking for an experienced Care Coordinator to join our friendly team at Willows Home Care.

Willows was founded in 2016 by Charlotte Wright after 23 years as a paramedic. Her goal was simple: build a home care company that puts people first – both the clients we support and the team who care for them. Today, we proudly support over 45 clients across our local community.

Charlotte now works alongside her daughter Beth, our Assistant Manager, and together they've created a business that still feels like family. We're not a large corporate provider – everyone knows each other, everyone helps out, and you're never just a number.

If you've worked in care coordination before, you'll know every day is different. It can be busy and sometimes unpredictable, but you'll never be expected to manage it alone. Charlotte and Beth still work alongside the team every day, helping with care calls, solving problems and supporting the office whenever needed. They understand the role because they still do it.

We believe in being honest from the start. The salary advertised is what you'll be paid, the hours are exactly as listed, and while overtime is available, it's never expected. When you're off, you're off.

Working Hours & Pay

Salary: £29,536 per year
Three fixed midweek office days (9:00am–5:00pm) based in Penwortham
On-call responsibility after office hours on your working days
Alternate weekend on-call rota (Friday evening to Monday morning), managed from home

The Role

As our Care Coordinator, you'll help organise the day-to-day running of our domiciliary care service, ensuring clients receive safe, high-quality care while supporting both our care team and office.

Like everyone here, you'll occasionally help with care visits when needed, so a full UK driving licence and access to your own vehicle are essential.

What You'll Be Doing

Coordinating care and managing schedules
Responding to new enquiries
Creating and reviewing person-centred care plans
Carrying out staff supervisions and supporting the care team
Speaking with clients and families to ensure high standards of care
Completing audits and risk assessments
Supporting the day-to-day running of the office
Working closely with Charlotte and Beth to continually improve our service
Providing care visits when required

Essential Requirements

Previous care coordination or similar experience
Level 3 Diploma in Health & Social Care (minimum)
Full UK driving licence and access to your own vehicle
Good computer skills and confidence using Microsoft Office and care management software
Be friendly, approachable and professional
Right to work in the UK
