Warwickshire County Council's Children with Disabilities Team is seeking a Family Support Workers to join our Review Hub on a permanent basis.

The Role
Working within the Children with Disabilities Review Hub, you will play a vital role in supporting children and young people with disabilities and their families. You will work collaboratively with children, parents, carers and partner agencies to review support arrangements, identify strengths, and ensure services continue to meet assessed needs and promote positive outcomes.
The team operates county-wide, with bases in both the north and south of Warwickshire. The successful candidates will be expected to travel across the county as required.
Key Responsibilities

Manage a caseload of children and young people who receive support packages to meet their assessed needs.
- Review packages of support and work collaboratively with families, providers and partner agencies to ensure they remain effective and outcome-focused.
- Coordinate and chair multi-agency review meetings.
- Visit children and families to review progress and assess ongoing needs.
- Provide advice, guidance, signposting and practical support to parents and carers.
- Undertake Carer's Assessments where appropriate.
- Work alongside Social Workers and support duty functions within the team.
- Maintain accurate and timely records in line with statutory and organisational requirements.
- Promote the voice of the child and support families to achieve greater independence and resilience.
About You
We are looking for individuals who:

Are passionate about improving outcomes for children and young people with disabilities.
- Have experience of working with children, young people and families.
- Can build positive relationships and engage effectively with a wide range of professionals and agencies.
- Have excellent communication, organisational and recording skills.
- Understand strengths-based and outcome-focused practice.
- Can work independently while contributing positively to a supportive team environment.
- Hold a full driving licence and have access to a vehicle suitable for business use.
