JOB TITLE: Advanced Nurse Practitioner
REPORTS TO: The Partners (clinically);
Practice Management Team (administratively)
HOURS: Dependant on contract
The post holder will work within professional boundaries in managing and providing high quality, holistic care to patients in a safe, multi-disciplinary environment through the provision of specialist clinics including chronic disease management, cervical smears, child/adult immunisations, contraception review and health education.
NMC requirements must be met and the individual will work within the NMC Code of Conduct to agreed practice standards and protocols.
Clinical Nursing Practice:
- The Advanced Nurse Practitioner works autonomously and is accountable for own professional actions.
- Undertake Clinical Nursing Practice at an advanced level, using expert knowledge and clinical skills, to deliver holistic care to people accessing primary health care services.
- Assesses diagnoses and treats patients attending primary health care services with a range of acute, non-acute and chronic medical conditions.
- Directly admits patients to secondary care hospital in acute medical need.
- Refer patients to other medical specialities for assessment if deemed appropriate.
- Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for x-ray.
Job Description: Advanced Nurse Practitioner
- Interprets a range of diagnostic tests and routine clinical procedures.
- Makes critical judgements of the highest order to satisfy the expectations and demands of the job e.g. first contact with patients presenting acutely and manages care in the presence and absence of protocols.
- Makes decisions where precedents do not exist, where appropriate without recourse to others e.g. direct referral of patients to Hospital Consultants without prior reference to other medical colleagues.
- Advises and supports others where standard protocols do not apply.
- Works within professional guidelines and codes of conduct.
- Works within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning, risk management.
- Extended and supplementary prescriber.
Care and Programme Management:
- Develops and manages highly specialised programmes of care and care packages and provides highly specialised advice concerning care normally beyond the scope of normal Nursing practice e.g. establishes differential diagnoses, management/treatment plans and makes referrals as appropriate.
- Communicates highly sensitive condition related information to patients, relatives and carers.
- Initiate the process of diagnosis with patients suspected to have a chronic disease e.g. diabetes, COPD, asthma, IHD referring to other clinical staff as appropriate.
- Contribute to practice targets both local and national within the disease management agenda e.g. QOF, prescribing incentive scheme.
- Contributes to disease registers.
- Audit outcomes of care against standards and initiate changes as necessary.
- Undertake annual review of patients’ understanding and ability to self-manage and medication reviews.
- Refers to other members of the extended primary health care team as appropriate e.g. chiropodist, dietician, specialist Nurses.
- Actively implement safeguarding protocols for children and vulnerable adults and to know who to refer to and how to escalate concerns about radicalisation.
Advanced Nurse Practitioner Strategic Development of Primary Care Services
- Contributes to the strategic development of primary care and public health services to meet patient and population needs within the practice and across the wider organisation and works/liaises across multi-professional and agency boundaries.
- Works at the forefront of practice innovation. Determines how to deal with ambiguous or unique problems to develop and advance primary care services for the benefit of patients, carers and communities.
- Recognition and involvement in the Maple Access Partnership as a teaching unit and help drive the Nursing element of training, including trainee NP’s.
- Develop links within the ICB and other local and national bodies relevant to primary care nursing initiatives.
- Initiates and carries out programmes of health screening and ensures other team members are aware of health agendas and screening programmes.
- Introduce innovation in practice to meet on going health need demands of the practice.
- Works to introduce new developments that will improve health outcomes for patients and is able to demonstrate through written and presentation these developments
- Share innovation and new learning with others.
- Supports practitioners in profiling the practice population in order to initiate contribute and promote improved health and prevent disease in individuals and groups.
- Represent individuals and families interests when they are not able to do so themselves.
- Make referrals to statutory and voluntary agencies as appropriate.
Management and leadership
- Work with the management team to deliver high quality patient care
- To lead the Nurse team
- Take an active role in clinical governance, particularly in clinical audit, guidelines and protocols
- To have an understanding of and work towards achieving the Quality and Outcomes
Framework and work with the lead nurse and management team to deliver this
- Contribute and access clinical supervision, reflection and peer review with the nursing team
- Lead and monitor training within the nursing team
- Provide evidence of ongoing personal development through appraisal and revalidation.
- Contribute to appraisal of other nurses in the team
- Attend and contribute to practice meetings, locality meetings and external collaborative meetings
- Maintain own professional development and registration in line with the Nursing and
Midwifery Council regulations
- Contribute in recruitment of other nurses in line with practice policy

- Critically evaluate and review innovations and developments that are relevant to the area of work
- Proactively promote the role of the advanced nurse practitioner within practice and externally
- Attend any training required in relation to management and leadership
- To play an active role in the development of the practice and new services including research.
- Contribute to a culture of ongoing review, taking into account new methods of working

Other duties, of a similar nature and appropriate to the grade, may be assigned from time to time to meet the changing needs of the practice.

Health & safety:
The post-holder will assist in promoting and maintaining their own and others’ health, safety and security as defined in the practice health & safety policy, the practice health & safety manual, and the practice infection control policy and published procedures. This will include:
- Using personal security systems within the workplace according to practice guidelines
- Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
- Making effective use of training to update knowledge and skills
- Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
- Actively reporting of health and safety hazards and infection hazards immediately when recognised
- Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holder’s role
- Undertaking periodic infection control training (minimum annually)
