Job Overview

Join our dynamic healthcare team as a Practice Nurse, where your expertise will directly impact patient well-being and community health. In this vital role, you will deliver high-quality nursing care within a busy primary care setting, supporting patients through compassionate, evidence-based interventions. Your proactive approach and clinical skills will help ensure smooth clinic operations, promote health education, and foster positive patient experiences. Duties

Provide comprehensive patient care including assessments, treatments, and health education tailored to individual needs
Assist with routine clinical procedures such as wound care, and health screenings
Update medical records accurately and efficiently
Support chronic disease management programs by coordinating follow-up care and patient education
Collaborate with multidisciplinary team members to develop personalized care plans
Maintain a clean, safe, and organized clinical environment in compliance with health and safety standards
Educate patients on health promotion topics, including lifestyle modifications and preventive measures
Manage DMARD Patients on regular basis.

Requirements

Proven experience in patient care within a primary or community healthcare setting
Excellent communication skills to effectively engage with diverse patient populations
Ability to work independently and as part of a collaborative healthcare team
Demonstrated attention to detail in documentation and clinical procedures
Current registration with the relevant nursing regulatory body (e.g., Nursing Council or equivalent)
Flexibility to adapt to changing priorities within a fast-paced environment
