Role Summary
We are seeking an organised, compassionate, and proactive Home Care Coordinator to oversee the day-to-day operations of our supported living homes. The Home Care Coordinator plays a vital role in ensuring that service users receive safe, effective, and person-centred care while supporting care staff to deliver high-quality services in accordance with CQC regulations and company policies.
The postholder will be responsible for coordinating the daily running of allocated supported living homes, supervising Care Workers, maintaining accurate care records, overseeing service user documentation, ensuring staffing levels are maintained, and promoting the wellbeing, independence, and safety of every service user.
Working closely with the Registered Manager, Line Managers, Operations Team, and external professionals, the Home Care Coordinator will ensure the service operates efficiently, remains compliant with regulatory requirements, and consistently delivers outstanding care.
Key ResponsibilitiesSupported Living Home Management
- Oversee the daily operations of allocated supported living homes, ensuring a safe, clean, and supportive environment.
- Monitor the wellbeing, safety, and individual support needs of all service users.
- Ensure all care and support is delivered in accordance with each individual's care plan and risk assessments.
- Promote independence, choice, dignity, and inclusion for all service users.
- Conduct regular welfare checks and monitor service user outcomes.
- Liaise with families, advocates, social workers, healthcare professionals, and commissioners where appropriate.
- Ensure safeguarding concerns are identified, documented, and escalated promptly in accordance with company policy.
Care Staff Coordination
- Coordinate and supervise Care Workers assigned to supported living homes.
- Delegate daily duties and responsibilities to care staff.
- Ensure staff understand and follow individual care plans and support requirements.
- Monitor staff performance, attendance, punctuality, and professionalism.
- Provide guidance, coaching, and day-to-day operational support to Care Workers.
- Assist with induction, shadowing, and ongoing staff development.
- Escalate staffing concerns, performance issues, or conduct matters to Line Managers.
Care Planning & Documentation
- Maintain accurate and up-to-date Service User files.
- Ensure Care Plans, Risk Assessments, Behaviour Support Plans, MAR charts, and daily records remain current.
- Complete regular reviews of care documentation.
- Monitor daily notes and ensure records are completed accurately and professionally.
- Ensure all incidents, accidents, safeguarding concerns, medication errors, and complaints are fully documented.
- Maintain Care Coordinator files and operational records in accordance with GDPR and company policies.
Medication & Clinical Oversight
- Ensure medication administration records (MAR Charts) are completed correctly.
- Monitor medication stock levels and report discrepancies immediately.
- Support medication audits and ensure compliance with company procedures.
- Escalate health concerns promptly to relevant healthcare professionals.
Quality Assurance & Compliance
- Ensure services are delivered in accordance with the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.
- Maintain compliance with CQC Fundamental Standards.
- Carry out regular audits of care records, service user files, medication records, and staff documentation.
- Participate in internal quality assurance processes and inspections.
- Support preparation for CQC inspections.
- Identify operational risks and implement corrective actions where necessary.
Staffing & Scheduling
- Assist in coordinating staffing arrangements to ensure safe staffing levels.
- Monitor rota compliance and report staffing shortages.
- Liaise with Care Coordinators and Operations regarding shift cover.
- Support emergency staffing arrangements where required.
- Ensure continuity of care across supported living homes.
Communication & Partnership Working
- Act as the first point of contact for Care Workers within allocated homes.
- Build positive relationships with service users, relatives, professionals, and external agencies.
- Attend multidisciplinary meetings where required.
- Provide clear handovers between shifts and management teams.
- Maintain effective communication across operational teams.
Health & Safety
- Ensure all supported living homes comply with Health & Safety legislation.
- Monitor environmental risks and report maintenance issues promptly.
- Ensure fire safety procedures, infection control practices, and emergency protocols are followed.
- Promote a safe working environment for staff and service users.
Administration
- Maintain accurate operational records and reports.
- Monitor staff training compliance and mandatory renewals.
- Assist with audits, spot checks, and service reviews.
- Complete weekly and monthly reports for management.
- Ensure confidential information is stored securely.

Qualifications & ExperienceEssential
- Minimum Level 3 Diploma in Health & Social Care (or equivalent).
- Previous experience within Supported Living or Domiciliary Care.
- Experience coordinating care staff or supervising care teams.
- Good understanding of safeguarding adults.
- Knowledge of CQC regulations and compliance requirements.
- Experience maintaining care plans and service user documentation.
- Excellent written and verbal communication skills.
Desirable
- Level 3 or 5 Diploma in Leadership for Health & Social Care (or working towards).
- Experience conducting audits and quality assurance checks.
- Experience working with adults with learning disabilities, autism, mental health conditions, or physical disabilities.
- Full UK Driving Licence.
Reporting Structure
- Reports directly to the Registered Manager / Operations Manager.
- Works closely with Care Workers, Line Managers, Compliance Team, and Senior Management.
- Liaises with external healthcare professionals, commissioners, social workers, and family members where appropriate.
