Job Title: Clinical Lead Case Manager – Complex Care & Mental Health (Children & Adults)

Location: Durham (Hybrid / Community / Office-Based)

Salary: (Pro-rata: £28,533 – £32,333 per annum for 22 hours/week, dependent on experience)

Employment Type: Part-Time (22 hours / week across 3 days – flexible working pattern available)

About the Role

Are you an experienced Registered Nurse (Paediatric, Mental Health, General, or Learning Disability) or Allied Health Professional with expertise in managing complex care across the lifespan? Looking for a flexible part-time role that offers true work-life balance?

We are seeking a Complex Care Clinical Lead Case Manager to join our team on a flexible 3-day schedule (22 hours per week). In this role, you will lead, guide, and oversee the clinical delivery of high-quality, person-centred care plans for children, young people, and adults presenting with complex medical conditions, physical disabilities, and co-occurring mental health needs across County Durham.

You will act as the primary clinical authority within a multidisciplinary team—driving high standards of holistic care, conducting age-appropriate clinical risk assessments, managing complex packages of care, and supporting service users, parents, and families through every step of their journey.

Key Responsibilities

Clinical Leadership: Oversee and mentor a team of case managers, nurses, and support workers, ensuring evidence-based clinical practices and regulatory compliance across paediatric and adult complex care packages.
Lifespan Holistic Assessment: Conduct comprehensive clinical, developmental, mental health, and risk assessments for children, young people, and adults with complex health needs and mental health conditions.
Care Coordination: Formulate and manage robust, individualised care plans in collaboration with NHS trusts, Child and Adolescent Mental Health Services (CAMHS), Adult CMHTs, social services, schools, and family care networks.
Transition Management: Lead and support smooth clinical transitions from paediatric services to adult complex care networks for young people reaching adulthood.
Crisis Management & Safeguarding: Oversee clinical crisis escalation protocols and manage complex behavioral, psychological, and medical risks while upholding robust child and adult safeguarding standards.
Quality & Stakeholder Engagement: Audit care delivery and build strong collaborative relationships with ICBs, local authorities, legal representatives, and external healthcare teams.

Requirements & Qualifications Essential:

Professional Registration: Active registration with the NMC (RSCN/Paediatric, RMN, RGN, or RNLD) or HCPC (Occupational Therapist or Social Worker).
Clinical Experience: Minimum 3–5 years of post-registration experience directly supporting both children and adults with complex medical needs and mental health conditions.
Specialist Knowledge:
Proven expertise in clinical medical interventions (e.g., long-term condition management, enteral feeding, respiratory/tracheostomy care, or severe physical/neurological disability).
Demonstrated background in mental health management and neurodiversity across age groups (e.g., trauma-informed care, risk mitigation, emotional health support, or Autism/ADHD support).
Safeguarding Knowledge: In-depth knowledge of both Level 3 Child Safeguarding and Adult Safeguarding practices.
Mobility: Full UK driver’s license with access to a vehicle for community visits across County Durham.

Desirable:

Dual qualification (e.g., RSCN/RMN or RGN/RMN) or postgraduate training in paediatric complex care or mental health.
Case Manager accreditation or membership with relevant professional bodies (e.g., BABICM or CMSUK).
Experience in transition pathways from children’s to adult health services.
