Full job description

An exciting opportunity has arisen to recruit a Band 7 Mental Health Pathway Lead within the Older Adult Outreach Team, working across the South Staffordshire West/East locality.

The Outreach Team provides a 7-day service (08:00–18:00), delivering a proactive and responsive approach to supporting older adults who require intensive intervention and home treatment as an alternative to hospital admission.

We are looking for a motivated and experienced practitioner with specialist knowledge in older adult mental health. The post holder will work with individuals experiencing a wide range of functional mental health conditions and dementia, including those with complex and challenging presentations.

This role combines clinical expertise with operational leadership responsibilities, working closely within a multidisciplinary team and alongside partners across physical health, social care, and primary care services

Deliver high-quality, person-centred care to individuals referred to the Older Adult Outreach Team
Provide day-to-day clinical and operational leadership, including supervision of staff
Support effective service delivery and contribute to team performance
Participate in Collective Leadership and Quality Improvement initiatives
Work collaboratively with MDT colleagues and external partners, including GPs and social care
Maintain accurate clinical records in line with organisational standards
Ensure compliance with mandatory training and take responsibility for ongoing professional development
