An exciting opportunity has arisen for a Senior Community Practitioner to join our friendly and dedicated team at Colburn Medical Practice.
This post would suit a Band 5 looking to develop community assessment and intervention skills.
The WestIntegrated Community Mental Health Team provides mental health services and delivers care to adults across Richmondshire and the Dales areas through two team bases. Our 2nd office is based in Leyburn.
As a member of the team, you will work alongside all members of the multi-disciplinary team which includes Registered Nurses, Social Workers, Psychologists and Psychology Assistants, Consultant Psychiatrists and medics, Team admin and support workers. With access to and support from specialist services available in the trust, and excellent working relationships with local partner organisations.

You must have the Right to Work in the UK in order to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.
As a key worker you will be expected to hold a case load of patients, to be able to deliver care to meet the identified patients needs autonomously and with the support from the MDT. You will be required to participate in the duty worker rota and support the patient journey from referral to discharge. As a senior community practitioner, you will be expected to provide support to junior members of the team and to provide an excellent learning environment for students who are placed with the team.
Candidates must have registration with the relevant governing body. Nursing candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework at the time of applying.
