We have a vacancy for a Social Worker in our Intermediate Care Multi-Disciplinary Team – a newly established social work team working throughout Carmarthenshire, based in Eastgate, Llanelli. You will be a part of a supportive team, committed to delivering high quality Social Work services and a wider service working together to best support adults maintain their independence and families in their caring roles.
To be successful, you will have an appetite for promoting independence, know what good practice looks like and be experienced in delivering excellent outcomes and high-quality interventions for the citizen group and their families at the front door. Candidates should work well in a demanding work environment and be able to manage competing demands whilst working in partnership with citizens, their carer’s, and other professionals, you will manage a caseload and practice in a person centred and outcome focussed way. Undertaking assessments, producing, and reviewing care and support plans will be fundamental elements of your role.
You will have a social work qualification, be conversant with the Social Services and Wellbeing (Wales) Act 2014 as well as knowledge and experience pertaining to relevant social work legislation and statutory obligations, role and responsibilities.
As a newly qualified Social Worker, you will have the best start with dedicated support to grow into your social work career. We also recognise the immense value of experience and the complex work experienced Social Workers undertake. You will have the opportunity to progress in social work practice (outside of a management pathway) via our ‘Experienced Practitioner Progression Framework’. Successful progression offers the higher range salary reward.
