Full job description

Job Summary

To ensure accurate and timely coding of clinical information into patient records using EMIS Web and supporting effective patient care, data quality, and compliance with NHS standards.

Key Duties:

· Read, review, analyse and process incoming clinical correspondence, both electronic and paper based

· Accurately assign SNOMED codes to diagnoses, procedures, investigations, test results and hospital correspondence

· Process and workflow documents within EMIS Web and docman to the appropriate clinicians and healthcare professionals

· Maintain high standards of data quality and record accuracy to support patient care, audits, QOF achievement and NHS contractual requirements

· Identify and resolve coding discrepancies in collaboration with clinicians and administrative colleagues

· Support the ongoing improvement of clinical coding processes and data quality initiatives within the practice

· Organise and maintain patient records, linking problem titles and ensuring consistency

· Workflow management: Direct clinical actions to appropriate team members based on correspondence content

· Ensure GP2GP transactions are processed in a timely manner

· Accurately summarise incoming patient records

· Occasionally provide break cover for members of the reception team when required

· Take a lead responsibility on an area of Chronic Disease Management for call and recall

· Understand the legislative requirements and practice policies for GDPR and Data Protection.
