Community Dementia Navigator
Location: Birmingham (community-based across localities with some office base and hybrid working)
Hours: 35 hours per week, Monday to Friday (occasional weekends)
Salary: £27,300 per annum
Contract: Fixed-term until 31 March 2028, with the possibility of extension

Age Connects West Midlands is seeking an experienced and motivated Community Dementia Navigator to join an ambitious city-wide programme designed to improve dementia awareness, diagnosis rates, and access to support across Birmingham's diverse communities.
This role is part of the Fairer Futures Fund programme, led by Birmingham and Solihull Mental Health NHS Foundation Trust, focused on reducing health inequalities and ensuring that everyone affected by dementia can access timely, culturally appropriate support.
We Are Looking for Someone With Dementia Experience
We are particularly keen to hear from candidates who have:
- Direct experience working with people living with dementia and/or their carers.
- Experience delivering dementia support, dementia navigation, dementia awareness, memory support, or community-based dementia services.
- Knowledge of dementia pathways, memory assessment services, and post-diagnostic support.
- Experience helping individuals and families navigate health and care services following a dementia diagnosis.
- A strong understanding of the challenges, stigma, and inequalities associated with dementia.
Candidates with backgrounds in dementia services, Admiral Nursing, dementia support work, social prescribing, older people's services, carers' services, memory clinics, community health, social care, or voluntary sector dementia programmes are strongly encouraged to apply.
About the Role
This is a unique opportunity to help shape how dementia is understood, identified, and supported across Birmingham.
Working within local communities, you will raise awareness, reduce stigma, improve access to diagnosis, and help ensure people affected by dementia receive the right support at the right time.
You will work closely with community organisations, healthcare professionals, Experts by Experience, carers, and local residents to improve outcomes for people living with dementia.
Key Responsibilities
- Deliver community outreach and engagement activities across Birmingham neighbourhoods.
- Lead dementia awareness, brain health, and myth-busting sessions.
- Build trusted relationships with community leaders, faith groups, and grassroots organisations.
- Facilitate focus groups, listening events, and engagement activities to understand barriers to diagnosis and support.
- Promote early recognition of dementia and encourage timely help-seeking.
- Support signposting and referrals into appropriate services.
- Co-design and deliver culturally appropriate post-diagnostic support initiatives.
- Gather community insight and contribute to service improvement and system-wide learning.
- Work collaboratively with health, social care, voluntary sector, and community partners.
Essential Skills and Experience
- Experience in community engagement, outreach, health promotion, social care, public health, or community development.
- Good understanding of dementia, including early identification, diagnosis, post-diagnostic support, and reducing stigma.
- Experience engaging and building trust with diverse and underserved communities.
- Strong communication, facilitation, and relationship-building skills.
- Experience delivering workshops, training sessions, consultations, or community engagement activities.
- Experience gathering, analysing, and reporting community insight.
- Ability to engage sensitively with individuals, families, carers, and communities on complex or emotional issues.
- Commitment to equality, diversity, inclusion, and reducing health inequalities.
Desirable
- Experience working directly with people living with dementia and/or carers.
- Knowledge of memory assessment services and dementia pathways.
- Experience working with NHS, primary care, or health and care services.
- Experience of co-production and working alongside Experts by Experience.
- Ability to speak a community language relevant to Birmingham's diverse communities.
