Job Summary

We are seeking a dedicated and compassionate Health Care Assistant to join our dynamic healthcare team. In this vital role, you will provide high-quality support to patients and residents, assisting with daily activities and ensuring their comfort and well-being. Your enthusiasm and proactive approach will help create a positive environment where individuals feel cared for, respected, and empowered. Responsibilities

Support patients or residents with personal care tasks such as bathing, dressing, grooming, and mobility assistance
Assist with medication management according to care plans and instructions from healthcare professionals
Maintain accurate records of care provided using IT systems and ensure compliance with confidentiality standards
Communicate effectively with patients, residents, family members, and healthcare team members to ensure clear understanding of needs and progress
Support the development and implementation of personalized care plans tailored to individual requirements
Drive or assist with transportation needs for appointments or outings when required
Contribute to a safe environment by adhering to health and safety protocols and reporting any concerns promptly

Skills

Excellent communication skills in English, with the ability to listen actively and convey information clearly
Strong IT literacy for documenting care activities, updating care plans, and using relevant healthcare software
Valid driving license is preferred for supporting home care visits or transportation duties
Experience working in a care home setting or providing home care services is advantageous
Knowledge of care plans, medication procedures, and health & safety standards in healthcare environments
Compassionate attitude with a genuine desire to support others’ well-being
Good organizational skills to manage multiple responsibilities efficiently
