Job summary
We are recruiting a Band 3 Occupational Therapy Assistant to join a newly established Community Rehabilitation Team (CRT) within Mid and North Hampshire Division.
This is an exciting opportunity to be part of a new, community-based rehabilitation service, supporting people with complex mental health needs to develop independence, improve daily living skills and achieve meaningful recovery.
You will work under the supervision of an Occupational Therapist to deliver practical, person-centred interventions with individuals in their homes and community settings.
The service focuses on functional recovery, helping individuals build the practical skills needed for independent living, including self-care, managing routines and engaging in meaningful activities.
The team works using an assertive outreach approach, delivering support in people's homes and community settings.
As an Occupational Therapy Assistant, you will:
Deliver structured 1:1 and group interventions to support recovery goals
Work alongside Occupational Therapists to implement rehabilitation plans
Support individuals to develop independence and confidence
Assist with home visits and community-based interventions
Contribute to service development and improvement activities, including audit
Work as part of the MDT to ensure coordinated, effective care

Main duties, tasks & skills required
The post holder will:
Deliver occupational therapy interventions as part of agreed care plans under supervision
Support individuals with daily living skills and community engagement (e.g. routines, social activities, accessing services)
Work 1:1 with service users in home and community settings, including lone working
Assist with and deliver group-based rehabilitation activities
Support service users to engage with recovery-focused interventions, including those who may find engagement difficult
Contribute to assessment processes by gathering information and reporting observations
Maintain accurate and timely clinical records
Support the day-to-day running of the service, including resources and admin tasks
Key skills required:
