Full job description

32 Hours per week / negotiable

We are seeking an experienced, motivated and compassionate Advanced Nurse Practitioner to join our friendly and progressive primary care team. The successful candidate will play a key role in delivering high quality, patient centred care to our diverse practice population, working independently and collaboratively within a multidisciplinary team.

The post holder will be responsible for the assessment, diagnosis, treatment, and ongoing management of patients presenting with acute & chronic health conditions. They will undertake comprehensive clinical assessments, develop and implement evidence-based treatment plans, request and interpret diagnostic investigations, and prescribe medications where qualified and authorised to do so.

This is an excellent opportunity for an ANP who is passionate about improving patient outcomes, promoting health and wellbeing, and contributing to service development within a forward thinking practice. The roles offers a varied and rewarding workload, including chronic disease management , health promotion, preventative care, and supporting the practice in meeting local and national healthcare priorities.

The successful applicant will demonstrate excellent clinical decision-making skills, and a commitment to continuous professional development, and the ability to work autonomously while maintaining strong collaborative relationships with GPs, nurses, pharmacists and other healthcare professionals. Independent prescribing or a willingness to work towards this, is desirable.
