North Bournemouth Primary Care Network is looking for an enthusiastic, proactive person with strong communication and organisation skills to join our friendly team as a Care Home & Service Support Care Coordinator, working across our practices.
The key aspects of this role will be coordinate and deliver the administrative functions of the Primary Care Networks’ Enhanced Health in Care Homes Service’ and provide comprehensive admin and support across other Primary Care Network services as required, including the Enhanced Access and Extended Hours Services.
This is a part time role of 20 hours per week, working on a Monday, Thursday, Friday (4 hours per day ) & Saturday (8 hours). It is hybrid working, with a combination of practice-based and home-based working on specific days. Due to the nature of working within a network, we are looking for someone who holds a valid driving licence with access a car
Full training and support will be given.

Enhanced Health in Care Home Service:
· Responsible for the Coordination and administrative support for all care provision within the PCN’s ‘Enhanced Health in Care Home Service.’
· Responsible for planning and managing the rota and appointments for the weekly Care Home ward round. Ensuring rota is finalised and uploaded on to patient data system at an agreed timeframe in advance.
· Ensuring that all medical records are kept up to date with any Enhanced Health in Care Home Service activities.
· Monitoring and actioning care home service-related tasks and emails.
· Liaison with Care Homes, making weekly ‘check-in’ calls to them and being the point of contact for all non-urgent care requests.
· Liaison with the PCN clinical staff within the Care Home service.
· Monitoring and ensuring that all new residents have a care plan in place within the agreed timeframes and ensuring that an annual care plan review/update takes place.
· Monitoring registrations from care homes across the practices within the PCN to ensure parity
· Reviewing vaccination status of all care home residence and liaising with practices where vaccinations are required.
· Co-ordinating vaccination campaigns for care homes where required and providing admin support to clinicians delivering the vaccines.
· Visiting care homes on a regular basis to strengthen relationships
· Attend Care Home team meetings as required.
· To undertake one weekday session per week working within practice premises, on a rotational basis across all sites, to support collaborative working and to develop and maintain effective professional relationships with individual practice teams around the care home service.
Enhanced Access and Extended Hours Services:
· To undertake working within practice premises on Saturdays, and to provide absence cover for Reception or Service Delivery Care Co-ordinator roles when required.
· Provide support when the Enhanced Access and Extended Hours Service Delivery Care Co-ordinator is absent to include (but not limited to):
o Ensure that any rota shortfall due to short notice cancellations is managed effectively, liaising with locum/agencies as required to ensure all shifts are covered and complete documented and stringent checking procedures to support provision of accurate and safe service delivery, e.g., GMC registration, DBS Checks.
o Ensure action is taken where possible to fill a vacant shift if a member of staff is unable to work.
o Dealing with any other issues that may arise to ensure smooth running of the service.
· Attendance at meetings as required.
Additional Responsibilities:
· Provide admin support for the weight management service, being a point of contact for all queries, forwarding on referral and collating and submitting monthly reporting returns and claims.
· Assist with other PCN projects as and when required
· Support other teams where directed by the PCN Board/PCN Management teams
· Attend regular monthly sessions working alongside the operations manager
· Attend PCN Care co-ordinators meetings

Experience
Demonstrate experience in an administrative role
Experience of communicating with all levels of professional staff and the public
Experience of dealing with confidentiality, sensitive data, appropriate maintenance and storage of records
Working experience of Microsoft Software Packages (Word, Excel etc.)
