Job Summary
We are seeking a highly motivated and compassionate Care Manager to oversee the daily operations of our homecare services, specialising in all sorts of adult health and social care settings. The successful candidate will be responsible for ensuring the delivery of high-quality care, maintaining compliance with regulatory standards, and leading a dedicated team of healthcare professionals. This role offers an opportunity to make a meaningful difference in the lives of clients, including those requiring specialised dementia and Alzheimer's care. The ideal applicant will possess strong leadership skills, hands-on experience in domiciliary and elderly home care settings, and a passion for providing person-centred care and support.

Duties

Oversee the overall care management of the company, ensuring smooth daily operations and high standards of care.
Develop, implement, and review personalised care plans tailored to clients' individual needs.
Supporting business growth by developing care packages and local partnerships
Supervise and support staff members, fostering a positive team environment through effective leadership and supervision.
Use company's Care Management System, Careberry for rota allocation and general care management processes.
Conduct Care Plan and Risk Assessments and draw up individual Care Plan for each client appropriate to their needs as soon as practicable following commencement of service.
Ensure compliance with all relevant legislation, regulations, and organisational policies related to adult social care such as CQC and Local Authority
Manage central medication administration system and EMAR processes safely and accurately in accordance with prescribed protocols.
Coordinate staff training programmes to maintain professional development and ensure best practices are adhered to.
Monitor clients’ wellbeing, addressing any concerns promptly while promoting dignity and independence.
Liaise with families, healthcare professionals, and external agencies to coordinate comprehensive care services.
Maintain accurate documentation relating to clients' records, incident reports, and regulatory audits.
Lead quality assurance initiatives to continuously improve service delivery for clients in their hones.

Requirements

Proven experience working within the domiciliary or other relevant care sector, with a strong background in elderly and complex care.
Relevant qualifications such as Level 5 Diploma in Leadership for Health & Social Care or equivalent is desirable. This role is ideal for a dedicated professional committed to delivering exceptional standards of elderly care that values compassion, professionalism, and continuous improvement.
Demonstrable supervisory or management experience within domiciliary care setting.
In-depth knowledge of dementia, learning disability and Alzheimer's care principles and practices.
Familiarity with developing and implementing personalised care plans.
Strong leadership skills with the ability to motivate and guide multidisciplinary teams.
Excellent organisational skills with attention to detail in record keeping and compliance documentation.
Knowledge of medication administration procedures and health & safety regulations relevant to adult social care.
Compassionate approach with excellent communication skills tailored to residents' needs and their families.
