Clinical Pharmacist (Maternity Cover)

The New Malden & Worcester Park PCN has an exciting opportunity for an experienced Clinical Pharmacist to join our dynamic multidisciplinary team on a fixed-term maternity cover contract commencing 3 August 2026.
This patient-facing role is integral to enhancing the quality of care for our patients through medicines optimisation, structured medication reviews, long-term condition management, and collaborative working with GPs and the wider healthcare team.
We are seeking a motivated, proactive and patient-focused Clinical Pharmacist who shares our commitment to delivering high-quality care. The successful candidate will join an innovative and supportive Primary Care Network and play a key role in maintaining continuity of care during a period of maternity leave. A comprehensive handover will be provided to support a smooth transition into the role.
Previous experience within general practice, a Primary Care Network, or a similar primary care setting is highly desirable. Applicants should be confident in undertaking patient-facing medication reviews, managing long-term conditions, and providing medicines optimisation support within a multidisciplinary environment.

Main Duties of the Role
The post holder will work as part of a multidisciplinary team to provide expertise in clinical medicines management, undertake structured medication reviews, support long-term condition management, manage medicines on transfer of care, and contribute to safer prescribing systems.
Patient-Facing Clinical Services
- Undertake structured medication reviews for patients with single or multiple long-term conditions.
- Optimise medicines to improve patient outcomes and reduce medication-related harm.
- Conduct clinical medication reviews and provide prescribing and monitoring recommendations to GPs.
- Carry out medication reviews for care home residents and housebound patients.
- Support multidisciplinary case conferences where appropriate.
- Provide telephone consultations for patients with medication-related queries.
Long-Term Condition Management
- Manage patients with chronic diseases where medicines optimisation is required.
- Support patients to achieve the best outcomes from their medicines through shared decision-making and personalised care.
Medicines Reconciliation and Transfer of Care
- Reconcile medicines following discharge from hospital, intermediate care services and care homes.
- Identify and resolve unexplained medication changes.
- Ensure continuity of medicines supply for vulnerable and high-risk patients.
Prescribing Support
- Manage repeat prescribing and reauthorisation processes in accordance with PCN policies.
- Ensure appropriate monitoring requirements are met for repeat medications.
- Respond to medicines-related queries from clinicians, practice staff, community pharmacies and patients.
Medicines Safety and Quality Improvement
- Identify patients at high risk of medication-related harm through risk stratification and clinical searches.
- Undertake prescribing audits and implement quality improvement initiatives.
- Support implementation of MHRA safety alerts and national guidance.
- Monitor prescribing against local and national formularies and recommend improvements.
Education, Training and Service Development
- Provide medicines optimisation education and training to members of the primary care team.
- Contribute to the development of new services involving medicines management.
- Support compliance with Care Quality Commission (CQC) standards relating to medicines.
Public Health and Collaborative Working
- Support public health campaigns and initiatives.
- Participate in multidisciplinary team meetings and collaborative working across the PCN.
Essential Qualifications
- Master's Degree in Pharmacy (MPharm) or equivalent.
- Current registration with the General Pharmaceutical Council (GPhC).
- Independent Prescriber qualification.
- Completion of relevant CPPE Primary Care training.
Essential Experience and Skills
- Minimum of two years' post-registration experience as a pharmacist.
- Experience of managing patients with acute and long-term conditions.
- Excellent communication and interpersonal skills.
- Ability to work effectively within a multidisciplinary team.
- Strong clinical reasoning and problem-solving skills.
- Understanding of medicines optimisation, prescribing governance and patient safety.
- Ability to prioritise workload and work independently within scope of practice.
Desirable Experience
- Previous experience working within General Practice or a Primary Care Network.
- Experience undertaking structured medication reviews.
- Experience managing long-term conditions within primary care.
- Familiarity with primary care clinical systems and prescribing processes.
