How you'll make a difference:
As an Adult Social Worker or Advanced Social Worker, you will play a key role in improving the lives of vulnerable individuals in our community, supporting them to live as independently and meaningfully as possible. You will work closely with people who need social care, along with their families and carers, to promote independence, ensure outcomes are clearly defined, and regularly monitored and reviewed.
The Hospital Social Care team is made up of social workers, occupational therapists, and social care practitioners. We apply the Three Conversations model to support individuals following hospital discharge, carrying out assessments and conversations in a range of settings, including care homes and rehabilitation units across the wider area. The team works collaboratively with both acute and community health partners and we have a small team based within the Transfer of Care Hub at Southmead Hospital, where we partner with other organisations to support multi-agency decision-making. We focus on a ‘home first’ approach while carrying out assessments to ensure the best possible outcomes for individuals.
What you will be doing:
- You will provide person-centred, strengths-based support, focusing on what matters most to individuals and what they identify will make the greatest difference to their lives. Using your initiative and creativity you will achieve the best possible outcomes.

It will be your responsibility to implement, review and monitor actions and outcomes, in line with individual care plans.

You will adapt and update plans where needed to reflect changing circumstances and evolving needs.

Working in line with relevant legislation, policies, and best practice guidance, you will uphold and promote the wellbeing, independence, and dignity of service users.

Using your professional knowledge and experience, you will make informed recommendations regarding the provision and use of reablement equipment.
Advanced Social Worker – Additional Responsibilities:

Drawing on research and evidence-based practice, you will lead on more complex and high-risk cases, applying advanced professional judgement and taking responsibility for key decisions.

You will contribute to workforce development, including supporting and supervising students on placement.

In line with advanced social worker expectations, you will maintain up-to-date core training and actively engage in continuing professional development.
What we need from you:
- It is essential you hold a recognised Social Work degree and be registered with Social Work England (SWE).

A strong understanding of asset-based and strengths-based approaches within adult social care is essential.

We require you to be a confident decision-maker and effective problem solver, with a genuine commitment to working collaboratively with individuals to achieve their goals and aspirations.

You will thrive in a fast-paced environment, remain open to challenges, adapt positively to change, and demonstrate excellent time management to work efficiently and meet service requirements.
Advanced Social Workers – Additional Requirements:

We are looking for someone who can bring at least 2 years post-qualification experience, including working with individuals with complex needs.

You must evidence advanced level practice, demonstrating high-quality professional reasoning, sound judgement and leadership in complex situations.

A clear commitment to Continuous Professional Development (CPD) is required.

- Salary is offered at points within these bands depending on experience.

Social Worker: £36,363 - £39,152

Advanced Social Worker: £40,777 - £44,075

This role will be based at our Badminton Road office, but you may also be required to work at Southmead hospital at times.

You will be expected to travel throughout the authority and surrounding area so must have a full valid driving licence with regular access to a vehicle, or have alternative appropriate means of travel.

Business insurance will need to be added to your insurance policy.

You can claim back mileage expenses.

Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post.
