Full job description

Meadowbanks Care Home is looking for a compassionate, reliable, and experienced Care Coordinator to join our dedicated team. This is a fantastic opportunity for someone who is passionate about delivering high-quality, person-centred care and is ready to take the next step in their career within a supportive and well-led home.

Hours

Early shift = 08.00 - 14.00
Late shift = 14.00 - 20.00
Long shift = 08.00 - 20.00
Night shift = 20.00 - 08.00

About Meadowbanks Care Home

Meadowbanks is an award-winning, 40-bedded residential care home situated within approximately four acres of beautifully landscaped grounds in Upminster.

We provide high-quality, person-centred care within a warm, luxurious and homely environment. Our focus is on promoting dignity, independence and wellbeing while ensuring every resident feels safe, respected, valued and genuinely at home.

About the Role

As a Care Coordinator, you will:

Coordinate each shift to ensure residents receive safe, person-centred care
Administer medication safely and accurately in line with residents’ care plans, prescriptions and the home’s medication policies
Complete medication stock checks, audits and accurate MAR documentation
Lead, support and provide guidance to care staff, allocating duties appropriately
Review and follow care plans, risk assessments and associated documentation
Support residents with meals and hydration, ensuring their nutritional needs are met
Monitor residents’ health, promptly reporting and responding to any changes
Complete effective handovers and ensure important information is communicated
Escalate safeguarding, clinical or operational concerns appropriately
Ensure staff follow the home’s policies, procedures and expected standards of care
Promote residents’ dignity, choice, privacy and wellbeing at all timesAbout You

We are looking for someone who:

Has previous experience as a Care Coordinator, Team Leader or Senior Care Assistant
Is confident in leading, supporting and motivating a care team
Remains calm, organised and professional when working under pressure
Has excellent communication skills and can work effectively with residents, relatives, colleagues and healthcare professionals
Is reliable, approachable and able to manage competing priorities
Has a genuinely caring nature and promotes dignity, respect, choice and independence
