We are pleased to invite applications for the position of Practice Nurse within our friendly, forward-thinking training practice, where staff wellbeing, professional development and clinical excellence are central to our ethos.
We are currently advertising for a Practice Nurse and/or Nursing Associate, however we will be appointing a candidate into a role of up to 35 hours per week or jobshare. The salary for this role is dependant on experience.
Our practice continues to grow in both scope and capability, with dedicated research facilities now established and enhanced clinical services available to patients within practice including diabetes specialist, rheumatology, and sexual health, minor operation and skin clinics.
Our clinical team is comprised of four Partners, three Salaried GPs and a skilled multidisciplinary workforce comprising Paramedic Practitioners, Advanced Nurse Practitioners and Practice Nurses with specialist expertise in heart failure and diabetes.
As a Practice Nurse at Readesmoor, you will play a pivotal role at the heart of our practice. You will work as an autonomous clinician, delivering exceptional, patient centered care while upholding the highest professional standards and supporting the wider nursing team.
The post holder will be responsible for clinical services including public health and screening programmes, infection prevention and control, chronic disease management, immunisations, health promotion, well man/woman clinics and cervical screening.
Main duties of the job
The postholder will support safe, effective patient care across infection control, longterm condition management, and population health.
Core duties include working to IPC policy, assessing patient needs, and contributing to quality improvement. The role develops and delivers care plans for frail or housebound patients and supports public health programmes such as immunisations, cervical screening and public health promotion.
Population health approaches are embedded to reduce inequalities and improve access.
The post holder will provide evidence based care, teach patients and families about condition management, and support new clinicians.
Responsibilities include participating in audits, updating protocols and working within national and local guidelines.
The postholder recognises professional boundaries, supports enhanced services, adheres to safeguarding legislation, and assists with complaints and learning from incidents.
The postholder completes mandatory training, contributes to clinical governance, review policies, and attend annual appraisal.
Additional duties include participating in public health campaigns, offering travel advice, providing wound care, maintaining accurate coded records, supporting QOF targets, managing chronic disease registers, maintaining a safe workspace, chaperoning, assisting in emergencies, and liaising with external agencies for vulnerable patients.
Other tasks may be required depending on workload and staffing.

Readesmoor is the largest GP surgery in Congleton, with a patient population of around 14,500. Congleton itself is a market town with a population of around 30,000. The patient population is varied in terms of its socioeconomic and geographic profile. Readesmoor is part of the Congleton and Holmes Chapel (CHOC) PCN, which is made up of 4 practices.
The PCN covers a population of around 47,000 across this area of Cheshire East. The PCN collaborate to provide services to patients, including coordinating vaccinations, extended access services and funding for additional primary care services and staff, such as Advanced Practitioners, Mental Health Practitioners and First Contact Physiotherapists.

Clinical Knowledge and Skills
Essential
- Experience of Practice Nurse skills including wound care, ECG, venepuncture, immunisations, new patient medicals and womans health (cervical cytology, contraception etc.)
- Request pathology tests and process the results, advising patients accordingly
- Understanding of the importance of evidence-based practice
- Ability to promote best practice regarding nursing matters
- Ability to work within own scope of practice and understand when to refer to GPs
- Good clinical system IT knowledge and an ability to record accurate clinical notes
- Ability to communicate complex and sensitive information effectively with people by telephone, email and face to face
- Understanding of safeguarding adults and children
- Understanding and knowledge of healthcare provision in GP surgeries, QOF and enhanced service
- Effectively able to communicate and understand the needs of the patient

Qualifications
Essential
- Registered Nurse (Nursing and Midwifery Council)
- Has evidence of working at the relevant level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework

- Mentor or teaching qualification
- Family planning qualification
Experience

- Broad knowledge of general practice and experience of working as a Practice or Community Nurse
