Job Title

Pharmacy Technician (GPhC Registered)

Hours

37.5 hours per week

Working across our three GP Practice sites (this is an on-site, patient-facing role and remote working is not available).

Working Hours

The successful candidate will work 37.5 hours per week, Monday to Friday, with shifts scheduled between 8:00am and 6:30pm.

Job Purpose

We are seeking a motivated and enthusiastic GPhC Registered Pharmacy Technician to join our friendly multidisciplinary team. This is a patient-facing role, working across our three GP Practice sites to support the safe, effective and efficient use of medicines and improve patient care.

The successful candidate will work closely with GPs, Clinical Pharmacists, Nurses and the wider practice team to provide medicines optimisation services, undertake medicines reconciliation, support repeat prescribing processes and respond to medication queries from patients and healthcare professionals.

This is an on-site position requiring regular face-to-face contact with patients and members of the multidisciplinary team. Remote or home working is not available for this role.

Main Duties and Responsibilities

Undertake medicines reconciliation following hospital discharge and other care transitions, updating patient records in accordance with practice protocols.
Support the safe and effective management of repeat prescribing systems and contribute to the continuous review and improvement of prescribing processes and Standard Operating Procedures (SOPs).
Respond to medication queries from patients, carers and members of the practice team, referring to a GP or Clinical Pharmacist where appropriate.
Provide a professional, patient-focused service through face-to-face consultations, telephone communication and clinical correspondence.
Work collaboratively with GPs, Clinical Pharmacists and other healthcare professionals to support medicines optimisation and prescribing safety.
Assist with medication reviews by preparing information, identifying prescribing issues and supporting follow-up actions.
Participate in prescribing audits and quality improvement initiatives to support safe, evidence-based and cost-effective prescribing.
Support the achievement of Quality and Outcomes Framework (QOF) and other contractual prescribing targets where appropriate.
Assist with monitoring prescribing budgets and liaise with the Medicines Management Team at Kent and Medway Integrated Care Board (ICB) where required.
Support the implementation of national and local prescribing guidance, medicines safety alerts and formulary changes.
Provide patients with appropriate medicines information and promote understanding of their prescribed medication.
Assist in the safe handling, storage and management of medicines within the practice.
Advise on the sourcing and availability of medicines where appropriate.
Assist with the submission of claims relating to personally administered drugs.
Maintain accurate, timely and confidential patient records in accordance with practice policies and data protection requirements.

Professional Responsibilities

Maintain current registration with the General Pharmaceutical Council (GPhC).
Undertake Continuing Professional Development (CPD) to maintain professional competence.
Organise and prioritise workload to meet agreed deadlines.
Work within professional standards, practice policies and current legislation.
Participate in mandatory training and practice meetings as required.
Maintain strict patient confidentiality at all times.

Health & Safety

The post holder will:

Follow all Practice Health & Safety policies and procedures.
Comply with infection prevention and control policies.
Keep work areas clean, safe and free from hazards.
Report health and safety concerns, incidents and near misses promptly.
Complete mandatory infection prevention and control training annually.
Use personal security systems and follow Practice security procedures where applicable.

Person Specification

Essential

GCSE (or equivalent) in English and Maths at Grade C/4 or above.
Qualified Pharmacy Technician (Level 3 or recognised equivalent).
Current registration with the General Pharmaceutical Council (GPhC).
GPhC registration number must be provided as part of the application.
Experience working in a patient-facing healthcare role.
Excellent communication and interpersonal skills.
Strong organisational skills with the ability to prioritise workload.
Good IT skills and experience using clinical computer systems.
High level of accuracy and attention to detail.
Ability to work effectively as part of a multidisciplinary team.
Ability to work across the Practice's three sites.
Ability to work on-site (non-remote).
Availability to work 37.5 hours per week between 8:00am and 6:30pm, Monday to Friday.

Desirable

Previous experience working within a GP Practice or Primary Care setting.
Experience of medicines reconciliation.
Knowledge of repeat prescribing processes.
Experience of prescribing audits and medicines optimisation.
Experience using EMIS Web or other GP clinical systems.
Understanding of Quality and Outcomes Framework (QOF) and Primary Care prescribing.
