Full job description

An exciting opportunity has arisen for a key member of senior staff within the Patient Safety and Assurance team. The role is diverse and touches upon a number of key Health Board functions including patient safety, incident management, assurance processes and regulator inspection action planning activity, including supporting clinical services to complete actions.

This role will support the assurance and governance of clinical / patient safety related to incidents, risks, external inspections and associated programmes of work.

As a senior member of the team, they will be required to deputise for the Patient Safety and Assurance Manager on occasion as required, ensuring continuation of the department’s functions.

The post holder will:

support the co-ordination and oversight of the management of the investigation process of all serious patient safety incidents and incidents of high impact to the organisation.
co-ordinate, undertake and advise on patient safety incident and other complex investigations or thematic reviews, including working with external reviewers.
manage the Duty of Candour process for allocated incidents.
provide specialist advice and support to the Triumvirate Team and others on all issues relating to risk and safety. This will include drawing on their own clinical and professional knowledge and extensive experience.
undertake patient safety incident and other complex investigations or reviews, within the scope of the service, ensuring they are managed in line with good practice and utilising established project management methodologies.
utilise advanced clinical skills and extensive knowledge base to undertake specialist assessments, critically analysing complex information to make informed decisions regarding material to inform investigations to ensure high quality investigations.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

Our services are provided in:
Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 99 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
Numerous mental health and learning disabilities services

Support the Patient Safety and Assurance Manager in the oversight of responses to external inspections and peer reviews including ensuring action plans developed in response are SMART.

Be responsible for the day-to-day management of their allocated caseload. This involves managing multiple complex investigations and programmes of work at any one time, with a substantial number of stakeholders.

Support the Quality Assurance and Safety Team through the Health Board’s governance structure, in identifying learning themes and feeding back the outcomes of investigations.

Undertake, review and analyse highly complex, sensitive investigations and interpret and apply Health Board policies and procedures, regulation and relevant statutory obligations.
