Job Purpose
The purpose of this role is to provide high-quality, person-centred care and support to individuals within their own homes, enabling them to maintain their independence, dignity, wellbeing and quality of life.
The post holder will deliver care in accordance with individual care plans, company policies, CQC standards and best practice guidance, while promoting choice, privacy, respect and safeguarding at all times.
Key ResponsibilitiesCare and Support
- Provide compassionate, person-centred support in accordance with individual care plans.
- Assist individuals with personal care including washing, bathing, dressing, grooming and toileting where required.
- Support individuals with continence care while maintaining dignity and respect.
- Assist with mobility, transferring and moving and handling procedures using approved techniques and equipment.
- Encourage independence and support individuals to maintain daily living skills wherever possible.
- Support service users with meal preparation, nutrition and hydration needs.
- Provide companionship and emotional support to reduce isolation and promote wellbeing.
- Encourage participation in social, recreational and meaningful activities.
Health, Safety and Wellbeing
- Observe, monitor and report changes in physical health, mental wellbeing, behaviour or care needs.
- Support individuals with dementia, learning disabilities, physical disabilities, sensory impairments, mental health needs and other support requirements.
- Recognise and report safeguarding concerns, accidents, incidents, falls, pressure damage and other risks.
- Follow risk assessments and contribute to the maintenance of a safe environment.
- Adhere to infection prevention and control procedures, including the correct use of PPE.
Medication and Record Keeping
- Support with medication in accordance with company policies, training and authorisation levels.
- Maintain accurate, timely and professional care records, daily notes and incident reports.
- Report concerns, changes and observations promptly to the appropriate manager.
- Ensure all information is recorded in line with confidentiality and data protection requirements.
Partnership Working
- Develop positive and professional relationships with service users, families, colleagues and healthcare professionals.
- Communicate effectively with all parties involved in the care and support of individuals.
- Promote equality, diversity, inclusion, dignity and respect in all aspects of care delivery.

- Care Certificate completion.
- NVQ/QCF Level 2 or Level 3 in Health and Social Care (or equivalent).
- Medication administration training and experience.
- Full UK driving licence and access to a vehicle.
- Knowledge of CQC standards and person-centred care practices.
