About us

An exciting opportunity has arisen for a Community Care Worker (CCW), join Warrington Borough Council's Localities Service.

We are working in partnership across the town to shape a health and social care system to be proud of, one that is fit for the future and built to last. We are looking for dedicated and motivated CCW’s who share our commitment to delivering high-quality, strengths-based and person-centred support to adults across Warrington.

The Localities Service consists of four geographical teams covering South, West, East and Central Warrington. Each team is supported by a Team Manager, Senior Practitioner, Social Workers, Community Care Workers and dedicated administrative support. As part of our supportive and experienced workforce, you will work alongside colleagues and partner agencies to help adults maintain their independence, wellbeing and quality of life.

We operate a hybrid working model, with a minimum of two days per week based at Time Square, Warrington, combined with visits to people in their own homes and other community settings.

We are committed to investing in our workforce and offer a wide range of learning and development opportunities. This includes access to our highly regarded Apprenticeship Programme, which provides a pathway to gain a Social Work Degree while remaining in paid employment. Successful completion of the programme leads to a secured Social Worker position, offering excellent opportunities for career progression.

The role

The Localities Service provides assessment, support planning and care management for adults living with a range of long-term and complex health and social care needs, including older adults living with frailty, dementia, sensory impairment and other significant health conditions.

Working within the Care Act 2014, Mental Capacity Act 2005 and other relevant legislation, you will undertake strengths-based assessments, develop personalised support plans and review ongoing care arrangements to ensure people achieve the outcomes that matter most to them.

Key responsibilities include:

Undertaking assessments of need and risk, including Mental Capacity Assessments and Best Interest decisions.

Developing and reviewing person-centred support plans that promote independence, choice and wellbeing.

Managing and reviewing care packages, including home care, direct payments and 24-hour care arrangements.

Completing safeguarding enquiries.

Supporting individuals who may be reluctant to engage with services, including people with complex needs and hoarding behaviours.

Participating in the team's duty rota, responding to referrals and urgent enquiries.

Working collaboratively with families, carers, health colleagues and partner agencies to achieve positive outcomes.

Maintaining a robust caseload with the support of regular supervision and a visible, approachable management team.

As a valued member of the team, you will have opportunities to develop your expertise across a wide range of practice areas, including safeguarding, Continuing Healthcare and risk assessment.
