Full job description

High View Care provides CQC regulated services in the areas of rehabilitation and wellbeing support for adults with acquired brain injury and other complex needs. The organisation has five residential units caring for 40 clients. It has been family owned and operated since 2004.
Successful candidates will be expected to work some evenings and weekends where required. They should be able to travel between units, ideally using vehicles provided.

Job Purpose
The purpose of the role is to engage with the service user (client) base - predominantly adults (aged 18 to 65) with acquired or traumatic brain injury to deliver positive and meaningful interventions in primarily home-like settings, all or mostly in the residential unit where the client is accommodated. The methodology will be:

Working with HVCS Therapeutic (TT) and Trans-disciplinary Team (TDT) to maximise residents (clients) wellbeing and rehabilitation
Providing practical and psycho-social care to each individual client based on an assessment of their condition, abilities and personal objectives.
Delivering a planned programme of therapeutic behaviour support aimed at securing improvements in cognitive / emotional / behavioural progress.
Assessing the progress being made by the client along the support pathway.
Collaborating with team members and other professionals as required to maximise sustainable improvements in clients’ wellbeing and rehabilitation.

Main Duties and Responsibilities

1. Implementation of Integrated Care Plans for each individual client (overseen by the Clinical Psychologist and/or Occupational Therapist).

2. Produce and implement activities across the week that will promote individual clients’ cognitive / emotional / behavioural progress.
3. Provide support and advice under the supervision of psychology/occupational therapy to clients and carers regarding:

Promoting independence with daily activities, e.g. meal preparation
Communication – expression, comprehension, reading, writing
Cognition – e.g. memory and attention
Emotional difficulties
Fatigue, stress/anger management
Work, education and leisure

4. Feedback on client progress and work collaboratively with the whole team in developing new working strategies. 5. Within the Residential setting work closely in collaboration with other members of staff to devise and implement the agreed strategies into everyday practice. 6. To strive to help each client gain the highest level of independence they can possibly achieve. 7. As required, data collection for the overall purpose of monitoring, reviewing and improving the standard of service delivered. 8. Receive six-weekly supervision from the Lead Clinical Psychologist or Occupational Therapist. 9. Complete in-house induction and such external training as decided by management.1 0. To participate in reviews with clients in order to develop mutually agreed outcomes for issues resulting from their physical/psychological impairment. 11. To be actively involved with colleagues, clients, carers, other professionals and agencies, to co-ordinate and implement agreed service provision. 12. To maintain accurate and concise case records and produce reports as necessary. 13. To contribute to the development of the NeuroRehabilitation Service as appropriate. 14. To maintain an up-to-date knowledge of medical conditions and treatment, disability issues. 15. To adhere to Company policies and practices, applicable legislation and relevant Codes of Ethics and Professional Conduct. 16. To promote awareness of the Service to other professionals and agencies. 17. To undertake any other duties as directed from time to time to meet the needs of the service. 18. The post holder will be responsible and accountable for carrying out the duties and responsibilities of the post with due regard to the Company’s Policies and Procedures.

GENERAL

A full, enhanced DBS disclosure will be necessary for this post.

QUALIFICATIONS/TRAINING

An upper second class honours degree or higher, in Mental Health or Mental Health Sciences, in Psychology, Occupational Therapy or related fields. Further education specifically in neuropsychology or neuro-occupational therapy

EXPERIENCE

Relevant experience of working at support level within Health & Social Care.
Experience of working within neuropsychology, occupational therapy or allied discipline providing ABI/TBI related interventions.
The ability to work with commissioners, service users and stakeholders (clinical and non-clinical) in relation to the Company requirements and Client needs.
Ability to interpret and contribute to person-centred support plans with relevance to the client group and strategic direction of Health & Social Care
Ability to work independently and remotely as required to self -manage and prioritise a varied workload.
Managing budgets and delivery of objectives and outcomes.
Working knowledge of the regulatory and statutory environment applicable to the Company and the Commissioners.
Ability to present complex information effectively (written and oral skills).
