A full UK driving licence and access to a vehicle is essential for this role.

Location: York (with occasional travel to Harrogate & Beverley)
Salary: Up to £32,000 per annum (depending on experience)
Hours: Full Time

We're looking for a passionate, organised and experienced Care Coordinator to join our growing team in York, working closely alongside our Registered Manager to ensure the highest standards of care and service delivery.

What You'll Be Doing

As our Care Coordinator, you will play a key role in ensuring our care services run smoothly every day. Your responsibilities will include:

Coordinating and maintaining care staff rotas
Scheduling care visits efficiently while ensuring continuity of care
Supporting the Registered Manager with the day-to-day running of the service
Liaising with clients, families, healthcare professionals and care staff
Responding promptly to changes in care needs and staffing requirements
Completing administrative tasks and maintaining accurate records
Assisting with the onboarding and support of new care staff
Conducting quality assurance (QA) visits to ensure the highest standards of care are consistently delivered
Completing service user reviews and working with individuals and their families to ensure care plans remain accurate and person-centred
Carrying out initial care assessments and reassessments, developing and updating care plans in line with individual needs and company procedures
Monitoring service quality and identifying opportunities for continuous improvement
Ensuring services remain compliant with CQC standards, company policies and regulatory requirements
Participating in the on-call rota where required
Travelling occasionally to our services in Harrogate and Beverley

What We're Looking For

Essential:

Minimum 2–3 years' experience in a senior support worker or care coordination role within domiciliary care.
Good understanding of CQC regulations and person-centred care
Excellent organisational and communication skills
Ability to manage multiple priorities in a busy environment
Strong IT skills and confidence using care management systems
Full UK driving licence and access to your own vehicle
Flexible, reliable and professional approach
Female applicant (Genuine Occupational Requirement applies)

Desirable:

Experience working closely with a Registered Manager
Knowledge of rostering software
NVQ Level 3 in Health & Social Care (or willingness to work towards)
