Full job description

NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non-clinical professions and roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.

The role:

This is an exciting opportunity for a band 6 Registered Nurse to work within the North West Rehabilitation Service. The post holder will have an excellent opportunity to work within a dynamic multi-disciplinary team providing assessment, treatment, and intervention to older people and disabled adults with multiple pathologies. The aims of the service include, promotion of independence, self management, admission avoidance and supporting hospital discharge. These duties are fulfilled in service users own homes and communities enhancing ability to maximise independence and deliver person centred care. The post holder will independently manage a complex clinical caseload within the service, applying specialist knowledge and skills relating to their nurse professional role as well as working as an effective and efficient partner in the interdisciplinary team.
Other duties will involve the supervision of support workers and students as well as providing professional nurse expertise and development in the multidisciplinary team.

You will be a RCN Registered Nurse. Experience of being part of a multi - disciplinary team and working in the community is desirable along with relevant experience in neurology, older peoples care, or rehabilitation.

This is a community post and you will work in a variety of locations in the North West of Glasgow City, therefore a full driving license is essential.

Duration, Location, and Working Pattern:

The role is based at Community Centre for Health, 547 Dumbarton Road, G11 6HU.
Fixed-term Contract for 9 months
Part-time position (26 hour per week), Less hours may be considered and can be discussed.
The shift pattern is Monday to Friday pattern with possible weekend cover as necessary

Key responsibilities:

Provide clinical leadership for the assessment of patient needs, implementation and evaluation of programmes of care, which are evidence based.
Utilise all available resources taking into consideration the lifestyle, gender and cultural background and ensure involvement with the patient, family, carers and significant others.
To assess capacity, gain valid consent and have the ability to work within legal framework with patients who lack the capacity to consent.
Acts as a clinical resource for the interdisciplinary team.
Act as a clinical resource for ward staff regarding complex discharges.
Maintain patient records in line with NMC guidelines for records and record keeping.
Use a variety of standardised patient assessments which require technical expertise in their execution and interpretation.
To be responsible for the prioritisation and delegation of nursing referrals/tasks within the service to junior and support staff, ensuring that patients are seen appropriately in line with professional and departmental standards.
To maximise functional independence and rehabilitation potential.
Manage an identifiable caseload of patients effectively and efficiently personally undertaking more complex cases.
Assess for and issue appropriate walking aid within remit
To assess stability with walking aid.
Maintaining home exercise and mobility programme.

Knowledge, training, qualifications and/or experience required to do the job:

First Level Registered Nurse with post registration experience.
Educated to degree level or working towards.
Relevant post registration qualification/ or relevant experience in a specialist area.
Ability to fulfil criteria outlined in the job description
Driver with full Driving license.
Demonstrates flexibility in working practice.
Knowledge of current best practice in chosen specialty
Experience or commitment to interdisciplinary working
Documented evidence of commitment to CPD
Advanced communication skills
Ability to work independently
Supervisory experience
Awareness of learning needs
