The role:
An exciting opportunity has arisen within the North West adult Learning Disability team for a Band 3 health care support worker. The team is comprised of a large multi-disciplinary team working to promote the health needs of adults with Learning disabilities and complex needs.
The post holder will work alongside the nursing and wider multidisciplinary team to deliver specialist, person centred care to clients accessing the service. They will assist in assessment of client’s needs, implementation and evaluation of interventions and carry out delegated tasks as per care plan, contributing to review of care planning documentation.
The post holder will function as part of a structured team with support from senior learning disability staff within the team. They will report and liaise effectively with all professionals involved in delegated work, maintain associated records. Interested candidates should have some experience of working in the health and social care sector.
The post holder will be supported to develop areas of interest in practice that complement the available skills mix, particularly within the nursing team and address the assessed needs of the client group.
Driving License is essential.

Key responsibilities:
You will be a key support to people whether at home, in hospital or other care setting. Assistance in enabling population resident within Glasgow City HSCP who have a primary diagnosis of learning disability to live a fully inclusive community life, minimising the impact of the learning disability or complex needs, and ensuring equal access to all health care and housing as other non-disabled residents. This role supports people where the need arises community and/or inpatient settings and close collaboration with third sector providers.

To ensure the programmes of care delivered to the clients are within an agreed framework and as delegated by the named professional
- To participate in the planning, development and facilitation of therapeutic group activities in collaboration with team members
- Develop rapport with individual clients, their carer’s/families, obtaining relevant information and encouraging their interest and involvement in their care planning
- To observe the general behaviour, ability and responses of clients, ensuring the timely dissemination of information with other team members
- To maintain clear communications, both written and oral, with a range of internal and external agencies

To contribute to establishing infrastructure which will promote quality learning disability services in East Renfrewshire HSCP Learning Disability Team and take forward the recommendations in The Keys to Life (re-launched 2019) and The Coming Home Report (2018) and Coming Home implementation 2022.

To support, advise and teach clients, relatives and care providers to give effective care, treatment and training in relation to designated programmes of care for clients
- To provide assistance with the provision of specialist learning disability advice, education and support throughout East Renfrewshire HSCP, working with primary care, mental health, community care and acute care partners.
Knowledge, training, qualifications and/or experience required to do the job:
Experience in working with people with a learning disability, or evidence of advanced learning. A high level of motivation, good interpersonal skills, resilience and positive role modelling.

Extensive experience of working directly with people with a learning disability and complex challenging behaviour.

Understanding of the impact of trauma on individuals, their families/carers and support staff.

Experience must include work with people with a learning disability
