East Cheshire NHS Trust is pleased to offer an exciting opportunity for three suitably experienced Band 4 Assistant Practitioners to join our newly established Neighborhood Rehabilitation Pathway. We welcome applications from individuals with a strong interest in working as part of an integrated community team.
There are three posts available across East Cheshire, with bases at Macclesfield, Congleton, Knutsford, Handforth and Poynton. The successful applicants will be expected to work flexibly and support colleagues across different sites within East Cheshire. As a result, the ability to travel efficiently between sites, including rural locations, is essential.
Applicants currently working at Band 3 with relevant experience are also encouraged to apply.
About the service
This is a new and developing service focused on supporting early discharge from hospital, enabling patients to regain their independence more quickly in the comfort of their own homes. We are a forward‑thinking and evolving therapy service, offering a wide range of developmental opportunities, from Therapy Assistant roles through to Advanced Clinical Specialist positions. Each professional discipline is supported by a designated professional lead, with strong links to GPs, Consultants and local universities.
Full and part time, and flexible working will be considered.
Hours per week: 37.5
We are looking for an enthusiastic and well-rounded candidate, with experience of working in the NHS.
The candidate should be motivated, with good communication skills and the ability to work within the therapy team and wider MDT. Organisation and time management is key to efficiently managing delegated caseloads.
It will be essential for the post holder to work under supervision of a qualified member of staff, as well as autonomously within trust policies and scope of practice and competencies, in addition to maintaining accurate and comprehensive patient records.
To work as part of a team providing a comprehensive, rehabilitation service for patients across East Cheshire. You will deliver therapy independently or jointly with the therapists in individual, group and multidisciplinary settings. This may include supporting aspects of rehabilitation care provision with our partner care agencies.
