If so, we have the perfect opportunities for you. North Yorkshire Council are recruiting for full time Social Workers within the Mental Health Team.
As a Social Worker you will work closely with a variety of teams and agencies, including health colleagues, stronger communities and prevention teams, voluntary and independent sector, promoting health and wellbeing and preventing, reducing, and delaying need, in line with the key principles of the Care Act. You will work with people in a person-centred way, utilising their unique strengths and celebrating individuality. Through strength-based assessments and interventions, you will support people with care and support needs and their carers to achieve their desired outcomes with a focus on their wellbeing. This will be reflected in the assessment and support plans that you complete in collaboration with the people you support, to ensure they are receiving meaningful interventions that make a positive difference to the lives they choose to lead. We believe that a person’s community and social networks play a vital role in ensuring that we achieve the best outcomes for people. It will, therefore, be part of your role to think creatively when looking for solutions for individual needs.
You will also have a good working knowledge of current adult social care legislation and application of the legislative principles, including the Care Act, Mental Capacity Act and Mental Health Act.
Mental Health Team:
As a Social Worker in the Mental Health Team, you will have responsibility for assessing, planning, implementing and evaluating social care for people in community and hospital settings. You will support people with Secondary Care needs, using a Recovery Model approach to help maximise people's potential to maintain their independence in their own homes and communities. You will work jointly with health partners to support people with a range of mental health needs, including working collaboratively to undertake s117 aftercare assessments, plans and reviews to reduce the risk of mental health deterioration and readmission to hospital. Experience working with people with complex and multifaceted needs would be beneficial but is not essential for these roles. We believe that a positive attitude, a tenacity for learning and a passion for supporting individuals to achieve their maximum potential are the desired attributes of applicants interested in undertaking this role. You will receive comprehensive training, support and guidance, and be given the opportunity to further your understanding of those supported by each specialist team. To be eligible for this post you will have a professional Social Care qualification in Social Work, DipSW or CQSW and have current registration with Social Work England.
About the Role

Working to improve the lives of individuals– issues tackled include self-neglect, hoarding, dependence on alcohol or drug misuse, homeless prevention and supporting individuals and their families living with dementia. We do all of this alongside specialists in these areas, providing the most appropriate social work intervention.
