Care Navigator – Woodhall Farm Medical Centre

Location: Hemel Hempstead
Salary: £12.71 per hour
Hours: Zero hours contract with a possibility of a permanent position

Are you passionate about patient care and excellent customer service? Woodhall Farm Medical Centre is seeking a Care Navigator to join our friendly and supportive team. This role is vital in ensuring patients receive the right care at the right time.

Essential Requirements:

Full UK Driving Licence.
Excellent customer care and communication skills.
Strong command of the English language, both written and spoken.
IT literacy, with proficiency in basic software such as Microsoft Word and Excel.
Ability to work effectively as part of a team and provide evidence of teamwork skills.

Desirable Skills & Experience:

Knowledge of EMIS (clinical system).
Previous experience working in a GP practice or similar healthcare setting.
