Care Coordinator - Peterlee

Location: Traynor Business Park, Peterlee, SR8 2RU
Salary: £30,000 + bonuses
Additional: Out-of-hours on-call (paid)
Company: Family-run care provider

About the Role

We are looking for a compassionate, organised, and experienced Care Coordinator to join our dedicated team. As a key member of our service, you will ensure that clients receive seamless, high-quality care that meets their individual needs. This is a rewarding role where you can truly make a difference in people’s lives.

Key Responsibilities

Coordinate personalised care plans by liaising with healthcare professionals, clients, and families
Schedule appointments and maintain strong communication with key stakeholders
Keep accurate, up-to-date records of client information and care plans
Monitor and review the effectiveness of care delivery
Provide emotional support and guidance to clients and their families
Work collaboratively with multidisciplinary teams to ensure holistic care
Line-manage a small team of care workers across Peterlee, Murton, Seaham, Blackhall and surrounding areas
Create and manage staff rotas for community-based care
Participate in an out-of-hours on-call rota (additional pay)

About You

Previous experience in care coordination or a similar role
Excellent communication and interpersonal skills
Strong organisational abilities with the capacity to prioritise and multitask
Competent in Microsoft Office and general administrative software
Compassionate, empathetic, and committed to providing exceptional care
Full UK driving licence and access to a car for work-related travel
Willingness to visit clients in the community as required
