Full job description

An exciting opportunity has arisen for an experienced HCSW to join our St Andrews inpatient rehabilitation team.

The Rehabilitation Service provide rehabilitation to the adult population within the 2 wards of the community hospital. Some patients will be followed up at home or homely setting or as an outpatient.

The primary function of this role will be to work with the Physiotherapy Service to deliver therapeutic treatment programmes to service users in the ward setting or their own homes, preventing hospital admission and supporting early discharge from hospital. Physiotherapists will provide specialist assessments to this patient group and allocate work to this post holder.

You will have an SVQ Level III in Health and Social Care or equivalent. Experience working as a Physiotherapy and/or Occupational Therapy support worker would be desirable. It would be desirable to have experience working with older people as they are the main group of service users referred.

Working as part of a supportive multidisciplinary team you will be expected to carry out ongoing treatment programmes for a mixed caseload of mainly older people.

You will have good organisational skills, excellent communication skills and be able to work independently as well as within the team.

There may be occasions where you will be required to work in one of the other community hospital sites.

Public Holiday working is in operation.

Our service offers regular supervision, PDP and supports training opportunities.

The ability to travel throughout the area is essential.
