Care Coordinator (Care Home Team)

Primary Care Network
Part-time: 18.75 hours per week

Are you an organised, proactive individual with excellent communication and IT skills? Do you enjoy working as part of a multidisciplinary team to improve the care of older people? If so, we'd love to hear from you.

We are looking to recruit a Care Coordinator to join our established Care Home Team, supporting approximately 850 residents across 30 care homes within our Primary Care Network, working across four GP practices.

This is a predominantly administrative role that plays a vital part in ensuring care home residents receive coordinated, timely and high-quality care.

About the Role

As Care Coordinator, you will be the central point of contact for many aspects of the team's work, helping to coordinate care and facilitate communication between care homes, GP practices and other healthcare professionals.

You will work closely with our multidisciplinary team, supporting the smooth running of clinical services and helping ensure residents receive the right care at the right time.

Main Responsibilities

Coordinate administrative support for the Care Home Team.
Act as a key point of contact for care homes, GP practices, residents and their families.
Liaise with clinicians and other healthcare professionals to coordinate resident care.
Manage referrals and follow-up actions.
Organise clinical visits, meetings and reviews.
Maintain accurate electronic patient records and clinical databases.
Monitor shared inboxes and respond appropriately to enquiries.
Support the coordination of vaccinations, annual reviews and other planned care initiatives.
Ensure information is communicated promptly and accurately between all stakeholders.
Provide general administrative support to the wider team as required.

About You

We are looking for someone who:

Has excellent organisational and time management skills.
Is confident using Microsoft Office and electronic clinical systems (training will be provided where required).
Has strong written and verbal communication skills.
Is able to prioritise a busy workload and work independently.
Has excellent attention to detail.
Is compassionate and professional when communicating with older people and their families.
Enjoys working collaboratively within a multidisciplinary team.
Can maintain confidentiality and handle sensitive information appropriately.

Previous experience within healthcare, general practice, care coordination or an administrative role is desirable but not essential.
