Join our dynamic Hospital Discharge Team and play a vital role in supporting safe, timely and person‑centred transitions from hospital to home.

Hospital discharge is often when people are at their most vulnerable. In this role, your decisions will directly shape outcomes for individuals, families and the wider health and care system – ensuring people leave hospital safely, with the right support in place.

The role:

You as the Social Care Assessor will support adults (18+) to leave hospital safely and regain independence
As a Social Care Assessor you will complete Care Act assessments including Needs, Risk, Mental Capacity and Best Interest decisions
As a Social Care Assessor you will use strengths‑based practice to deliver flexible, outcome-focused support
As a Social Care Assessor you will apply Prevent, Reduce, Delay principles to minimise long-term care dependency
As a Social Care Assessor you will work on-site as part of a Multi-Disciplinary Team (MDT), with occasional home visits
As a Social Care Assessor you will collaborate closely with nurses, therapists and other health professionals
As a Social Care Assessor you will manage a varied caseload, prioritising early intervention and short-term support
As a Social Care Assessor you will commission services including reablement, domiciliary care, assistive technology and carer support
As a Social Care Assessor you will take part in MDT meetings, duty functions and shared decision-making
As a Social Care Assessor you will ensure all work is compliant with legislation, policy and best practice
