Your Role

As our Care Coordinator, you will be at the heart of the service.

You will help ensure that every client receives safe, reliable and consistent care from the right caregiver.

You will build rotas, respond to changes, communicate with families, support caregivers and solve problems before they become bigger concerns.

You will work closely with the Registered Care Manager and the wider office team to maintain high standards and deliver an excellent experience for everyone who depends on our service.

Your responsibilities will include:

Creating safe, effective and well-organised care rotas
Matching clients and caregivers based on skills, personality, location and individual needs
Protecting continuity of care and familiar caregiver teams
Responding calmly and quickly to sickness, emergencies and last-minute changes
Supporting caregivers with questions, concerns and daily challenges
Communicating warmly and professionally with clients and families
Monitoring punctuality, attendance and care delivery
Ensuring important information is communicated clearly
Keeping records accurate and up to date
Supporting quality monitoring and service improvement
Working closely with the Registered Care Manager
Participating in the on-call rota

Some days will be busy. Some situations will be challenging.

Essential Requirements

Experience in health, social care or home care
Excellent verbal and written communication skills
Strong organisation and attention to detail
Confidence using computer-based systems
Ability to manage a busy and changing workload
Willingness to participate in the on-call rota

Desirable

Previous Care Coordinator or rota coordination experience
NVQ or QCF Level 3 in Health and Social Care
Experience supporting or supervising care staff
Driving licence and access to a car
