Care Co-Ordinator
Are you an experienced administrator or current Care Co-Ordinator looking for a new challenge? The Ware & Rurals PCN is seeking an enthusiastic individual to join our team at the Buntingford & Puckeridge Medical practice.
Care coordinators provide extra time, capacity and expertise to support patients in preparing for or in following-up clinical conversations they have with primary care professionals. They will work closely with the GPs and other primary care professionals within the PCN to identify and manage a a caseload of identified patients, making sure that appropriate support is made available to them and their carers, and ensuring that their changing needs are addressed. They focus delivery of the comprehensive model to reflect local priorities, health inequalities or population health management risk stratification. Care coordinators will have a base Practice, but may be expected to engage with patients from both Practices within the PCN, seeing patients either in the Practice, care homes or at the patient’s home where this is appropriate.
Main Responsibilities: · Working towards achievement of Practice and PCN directed priorities (e.g. Cancer screening, Immunization programmes and QOF). · Proactively identify and work with a cohort of people to coordinate and navigate the support necessary for their personalised care requirements · Support patients by answering their queries and supporting them to make appointments · Providing good quality information to help patients make choices about their care · Ensure regular and consistent communication with the patient, referrer and wider care system regarding patient progress and any complications or guidance · Attendance at and coordination of practice level GSF meetings · Provide support to patients to take up national screening programmes
· Support patients in the uptake of local and national immunization programmes · Within a defined scope of priority areas, monitor referrals to ensure tasks are completed and care delivered in line with national timelines · Direct liaison with multi agencies to coordinate pathways of care for patients · Provide coordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers and other primary care roles
Work with patients identified as Carers – signposting to additional services · Refer to PCN social prescribing link workers were a patient is identified as potentially benefitting from this service · Support Quality and Outcome Frameworks and other DES/LES specifications · Work within all policies and procedures ensuring that individual's and carers' information remains confidential · Provide accurate and timely performance reporting within the agreed framework
