The position involves sourcing care and support in response to an identified care plan, maintaining effective information about available services to support the care management and commissioning process, including communication with service users, families, and professionals. What we are looking for:

Experience of working in multi-agency and multidisciplinary setting and systems
Experience of working in a fast paced and pressured environment
An understanding of care coordination
Experience in using Liquid Logic and ContrOCC
The ability to co-ordinate support using a wide variety of care options e.g. direct payment, commissioned service, community sector organisation
Excellent written and verbal communication skills
A Strong team player across own and other teams internally and externally, actively contributing to the positive development of the team
Commitment to excellent customer service
