Full job description

We have an exciting opportunity to join us as a Client Relationship Manager & help deliver the best experience to our clients.

You will be responsible for the following:

Being the point of contact for Clients & their families, from their initial assessment, through to their quality assurance reviews & any day to day queries or needs.
Delivering an unparalleled level of communication & customer service to all clients.
Support a high quality, seamless start to clients care by selecting the right Care Assistants to support them & allocating them to your client, as well as handing over to the wider team about the clients needs.
Build & maintain your own portfolio of Referral Partners, to generate enquiries and collaboration opportunities.
Attend local events to build our brand & find opportunities to connect with relevant community partners.
Deliver faultlessly on client follows up including Quality Assurance Reviews, Enquiry Follow Ups & Actions set during client meetings.
Advocate for them & liaise (where consent is given) with their wider network of Healthcare Professionals.
Writing of person centred care plans

A bit about you:

You're a people person & enjoy being around clients, colleagues & community partners.
Able to report on activities and understand the need to deliver output (Being productive not just busy)
Love delivering on targets & deliverables
Take pride in the quality & detail of your work
You value the privilege we have, being trusted to look after over 130 people locally
Good IT skills - Including Office 365
Enjoy meeting new people & sharing your passion for what you do

The role requirements:

You're able to work a set roster which includes weekend working whilst on call
Your standard shifts will be 08:30 to 17:00 on weekdays
You will be required to work 1 late shift per week of 14:30 until 22:30
You're able to work flexibly in line with the needs of the business
You're able to support the On Call roster, taking the On Call 1 in 5-6 weeks (linked to your weekend to work) so our clients and Carers are supported
You have access to a car & are happy working independently in the community
You're able to provide Personal Care & Support visits to clients
You will be able to manage a small team to ensure that KPI’s are met

Competitive salary - £27,040 plus additional earnings of around £1,300 On Call as well as annual performance based bonus.

As a Client Relationship Manager for Right at Home Chippenham, you will play an integral part in managing the daily communications to your portfolio of clients, ensuring they have maximum continuity with their care visits & enabling us to help more people than ever. You must be extremely personable as you will be working with Care Assistants, Clients & their families to make sure everyone is supported, connected & happy. This role is extremely fast paced and requires someone truly dedicated and able to remain positive and professional when working under pressure.
