To assess for and provide, brief focused therapy (using a range of interventions and models) to clients referred to the Mental Health In Reach service (MHIR).
To provide specialist clinical services within the Mental Health Team including clinical supervision, group interventions and training to colleagues or partner organisations and to work in accordance with NHS local and national protocols and your own professional body’s Code of Ethics.

Clinical:
To undertake assessment of clients referred to the service, including assessment for appropriate psychological intervention and assessment of risk.
To provide therapeutic interventions to clients of the MH team within agreed time frames.
To provide interventions for a variety of service users including those with common mental health problems and severe and enduring emotional, behavioural and mental health difficulties.
To manage your caseload effectively and to maintain adequate records of clinical work.
To undertake psychological assessments and clinical assessment and outcome measures with clients.
To undertake an assessment of risk and to respond to client crisis as appropriate.
To use theory, evidence-based literature and research to support best practice in clinical work.
To work with clients to develop a joint understanding of their difficulties and support them in accessing appropriate interventions.
To provide information on available treatment options for people with mild to moderate and severe mental health problems where appropriate.
To support clients nearing release in accessing statutory and voluntary community services where appropriate.
To promote self-help, self-management and healthy lifestyles enabling people to build their emotional health and capacity to deal with life events.
To monitor treatment and client progress and provide appropriate feedback to clients, supervisors and multi-agency colleagues where appropriate.
When necessary, to make referrals to appropriate agencies in consultation and communication with the client and to liaise as appropriate with those agencies.
Service Development:
To offer support and mentoring to assistant psychologists, STR workers and therapists on trainee placements.
To attend clinical supervision and CPD on a regular basis and as agreed with the Service Manager.
To attend and assist in providing group supervision where required.
To identify training needs to more effectively meet the therapeutic needs of the population where you work.
To assist in the evaluation of the service by contributing to data collection and analysis and to participate in research as required.
To actively contribute to the planning and development of the service.
To work with the team to initiate and develop quality improvement and innovative projects within the mental health team.
To use base Establishment and Trust IT systems to an adequate level to comply with the Trust’s clinical governance framework and to ensure accurate records are kept of patient attendance.
To participate in audit and evaluation as required by the service.
To communicate with the Mental Health Team about individual clients, using the practice’s IT system, team meetings, or other communication such as letters.
Communication:
To develop and maintain effective professional relationships with service-users and other professionals involved in their care e.g GPs, practice nurses, counsellors.
To accept referrals from other professionals and to communicate with them about progress verbally and in writing as required.
To provide and receive complex and emotive information to individuals, carers, members of the public and professionals and react accordingly with sensitivity to mental health issues (working within Caldicott Guidelines), individual circumstances and beliefs.
To work within administration protocols for contacting patients, arranging appointments and collection of patient data.
To keep accurate and confidential patient notes and maintain the highest standards of clinical record keeping, handling all patient information in line with Trust Clinical Governance Policy.
To attend relevant service meetings, to ensure effective liaison and information exchange.
To communicate and liaise with staff from the Commissioners, Provider Trusts and Community organisations to promote positive links.
Individual Practice, Training and Supervision :
T o work independently on a day-to-day basis, time managing the clinical and service development aspects of the job.
To receive regular supervision for all clinical work from a supervisor.
To receive regular line management for all other work related to service development and balancing the needs of client work, administration, data entry and CPD.
To complete all compulsory Trust training and participate in a programme of training and professional development in order to develop clinical knowledge and practice relevant to the role.
To contribute to the training and support of other primary care staff and the general public in primary care and community settings in dealing with mental health issues and promotion preparing and developing training materials as required.
To maintain accreditation with appropriate professional / regulatory body.
Other:
To participate in the Trust’s appraisal system and take responsibility for the development and maintenance of the highest personal standards of practice and professional self-governance, through active participation in internal and external CPD in agreement with the service manager.
To undertake other duties and responsibilities appropriate to the grade of the post as agreed with the service manager.
To ensure all work undertaken is within the framework, objectives and principles of Clinical Governance.
To recognise personal and professional limitations and not overstretch own capacity.
To work within the employment Terms and Conditions of Service of Central and North West London NHS Foundation Trust
