Full job description

We’re looking for a motivated and enthusiastic Occupational Therapist to join our Intermediate Care service, working across community and hospital pathways to support people to regain independence and return home safely.

This is a great opportunity for a newly qualified or experienced Band 5 Occupational Therapist to develop their skills within a supportive, forward-thinking team. You’ll manage your own caseload , completing assessments, delivering interventions , and working closely with colleagues across health and social care to achieve the best outcomes for the people we support.

You’ll be supported to grow as an autonomous practitioner, with access to regular supervision , peer suppor t, and development opportunities tailored to your experience. For those earlier in their career, this offers a strong foundation to build confidence and clinical skills ; for more experienced Band 5s, there are opportunities to further develop clinical expertise and contribute to service development and support improvements in pathways .

As a Band 5 Occupational Therapist within the Intermediate Care service, you will manage a varied clinical caseload, supporting people to regain independence and achieve safe and timely discharge from hospital or community pathways.

In this role, you will:
Manage your own caseload

Carry out holistic assessments and identify needs and risks

Plan and deliver person-centred interventions

Work closely with the wider team to support discharge and flow

Prioritise work and respond to changing demand

Keep clear, accurate clinical records

Delegate appropriately to therapy assistants and support their development

We are looking for someone who:
Is motivated, compassionate and committed to promoting independence

Has strong communication skills and is confident working with patients, carers and professionals

Can manage their own workload and prioritise effectively in a fast-paced environment

Demonstrates sound clinical reasoning and a willingness to learn and develop

Works well as part of a team and contributes positively to service delivery

Is flexible and able to adapt to the needs of the service

You’ll play a key role in supporting people at a critical point in their recovery, making a real difference to outcomes and helping people return to what matters most to them.
