JOB TITLE: Care Coordinator

RESPONSIBLE TO: PCN Service Manager

ACCOUNTABLE TO: PCN Clinical Director

PAY: £25,147.00-£27,596.00 per year

HOURS: 37.5

BASE: Work flexibly across 4 member practices (all within 10 minute distance)

SCOPE OF THE JOB

This new and exciting role will work with the Lea Valley Health PCN team supporting a local population to provide high quality and proactive healthcare services.

The Care Coordinator role is an exciting opportunity to play a pivotal role in supporting people by coordinating their health and social care services across a range of partners within our local Integrated Care System (ICS). The initial focus of this role will centre on coordinating Cancer Care for those patients who either need to access screening services or subsequent clinical appointments. Lea Valley Health PCN’s vision is to develop the Care Coordinator role to include a broader remit of care coordination including vaccine + immunisation programmes as well as ensuring all our patients have best practice care aligned to NHS England’s Quality & Outcome Framework (QOF)

Main Responsibilities:

· Working towards achievement of PCN directed priorities (eg Cancer care, Immunization programmes and QOF).

· Proactively identify and work with a cohort of people to coordinate and navigate the support necessary for their personalised care requirements

· Help people to manage their needs, answering their queries and supporting them to make appointments

· Ensure that patients have good quality information to help them make choices about their care

· Ensure regular and consistent communication with the patient, referrer and wider care system regarding patient progress and any complications or guidance

· Attendance at and coordination of practice level GSF meetings

· Have some in-sight into and coordinate /support patients uptake of national screening programmes

· Have some in-sight into and coordinate / support patients uptake of local and national immunization programmes

· Within the scope of PCN priority areas, monitor referrals to ensure tasks are completed and care delivered in line with national timelines

· Direct liaison with multi agencies to coordinate pathways of care for patients

· Provide coordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers and other primary care roles

· Refer to PCN social prescribing link workers were a patient is identified as potentially benefitting from this service

· Support Quality and Outcome Frameworks and other DES/LES specifications

· Work within all policies and procedures ensuring that individual's and carers' information remains confidential

· Provide accurate and timely performance reporting within the agreed framework

Other points for discussion:

· Maintain and develop engagement with all practice staff and encourage ‘best practice’

· Act as the first port of call for patients, in their caseload in relation to their care.

· Bring together all of a person’s identified care and support needs, and explore their options to meet these into a single personalised care and support plan (PCSP), in line with PCSP best practice

· Support people to understand their level of knowledge, skills and confidence – their “Activation “level – when engaging with their health and wellbeing, including using the Patient Activation Measure

· Explore and assist people to access personal health budgets where appropriate

· Support the coordination and delivery of MSTs (Multi-specialist Team meetings) within the network.
