Full job description

A well-established medical practice in Winsford is seeking an enthusiastic Clinical Pharmacist to provide high-quality support to our patients and healthcare team. The ideal candidate will be an independent prescriber with primary care experience in performing medication reviews and managing prescriptions. The role offers training opportunities and a supportive team environment. Applicants should demonstrate excellent communication skills, attention to detail and the ability to work independently within a multidisciplinary team

You'll be embedded as a valued team member at Swanlow Medical Centre, working alongside our current experienced IP Pharmacist. You'll bring your clinical knowledge to bear every day, leading on prescribing, owning medicines optimisation, and building the kind of trusted relationships with GPs and patients that make a Pharmacist genuinely indispensable.

Salary: £55,000 - £58,856, dependent on experience for 37½ hours, however we are looking for between 15 and 25 hours per week, hours negotiable across Mondays, Thursdays and Fridays.

Main duties of the job
As one of the resident medicines’ experts at Swanlow Medical Centre, you'll engage with the full range of clinical pharmacy work that keeps a busy general practice running safely and efficiently.

· Undertake complex structured medication reviews and polypharmacy optimisation

· Manage medicines for long-term conditions under agreed clinical protocols.

· Provide independent prescribing, deprescribing and medicines optimisation.

· High-risk drug monitoring.

· Support repeat prescribing and medicines reconciliation following hospital discharge, which may include home visits.

· Support CQC compliance relating to medicines and safer prescribing systems.

· Oversee medicines-related risks, audits, and significant events.

· Act on MHRA alerts, NICE guidance and safety notices relevant to general practice

· Maintain oversight of prescribing safety, repeat prescribing and medicines-related risk

· Own and update medicines-related risks on the practice risk register

· Own medicines-related significant event analyses and learning and share this with the practice team as necessary.

· Provide expert advice to clinicians on prescribing and medicines optimisation

· Ensure practice prescribing is aligned to ICB formulary and local policies

· Act as a key practice link for ICB medicines optimisation teams

· Lead or contribute to ICB-requested audits, improvement work and reporting

· Attend and contribute to practice meetings as appropriate

· Represent the practice at medicines-related system meetings where required

· Ensure safe systems for prescription authorisation and clinical oversight

· Contribute to QOF delivery, support care home patients with their medication needs, and play an active role in reducing medicines-related harm and unplanned hospital admissions.

Pay: £55,000.00-£58,856.00 per year
