Practice-Based INT Care Coordinator

Location: Montgomery House Surgery, Bicester Health Centre & Alchester Medical Group
Hours: Full-time – 37.5 hours per week
Salary: From £24,935 - £26,572 (depending on experience and qualifications)

About the Role

We are looking for a compassionate, organised, and proactive Care Coordinator to join our Integrated Neighbourhood Team (INT) within the Primary Care Network (PCN).

This is a rewarding role supporting patients with complex or long-term health needs to receive joined-up, personalised care. You will work closely with GPs, community teams, hospitals, social care services, and patients’ families to help coordinate care and improve patient outcomes.

The role is based across three practice sites with some flexibility in working hours. Travel between sites and occasional home visits are required, so access to your own transport is essential.

What You’ll Be Doing

Coordinating Patient Care

Support patients following hospital discharge or those at risk of hospital admission
Help develop and update personalised care plans
Carry out patient assessments by phone, in practice, or during home visits
Communicate with patients, carers, and healthcare professionals to ensure coordinated care
Signpost patients to appropriate health, social care, and community services

Supporting Multidisciplinary Team (MDT) Meetings

Attend and help coordinate daily virtual MDT meetings and weekly wider team meetings
Prepare agendas, gather patient information, and track follow-up actions
Work closely with GPs, nurses, therapists, pharmacists, social prescribers, and community teams

Administration & Data Management

Maintain accurate patient records using EMIS and other IT systems
Support referrals, tests, and follow-up actions identified by the MDT
Assist with data collection and reporting linked to service outcomes and contracts

What We’re Looking For

We’re looking for someone who is:

Highly organised with excellent communication skills
Compassionate and confident working with vulnerable patients
Able to manage a varied workload and work independently
Comfortable working as part of a multidisciplinary team
Skilled in administration and IT systems, including Microsoft Office
Flexible, motivated, and solution-focused

Essential Requirements

Relevant administration, healthcare, or care coordination experience
Strong interpersonal and organisational skills
Ability to maintain confidentiality and handle sensitive information appropriately
Access to a car and ability to travel between sites and patient homes
Willingness to undertake ongoing training and development

Desirable Experience

NHS, healthcare, or social care experience
Knowledge of long-term conditions and care planning
Experience working in multidisciplinary teams
Understanding of health and social care systems
Familiarity with medical terminology and EMIS systems
