Introduction
Some young people aren't ready to walk into a classroom. Not because they lack ability, but because life, anxiety, previous experiences of education, or their additional needs have made learning feel difficult, overwhelming or unsafe. That's where this role begins.
We're looking for a Transition Support Tutor to work with young people aged 16–25 who need a more personalised route back into learning. Many of the learners we support have an EHCP and face barriers linked to SEND, SEMH needs, anxiety, autism, trauma, confidence, or life circumstances that have disrupted their education.
You won't always start your work with a learner in a classroom or centre environment. Sometimes you'll meet them at home. Sometimes in the community. Sometimes you'll spend weeks building trust before formal learning becomes possible.
The role is about helping young people take the next step – whatever that step looks like for them.
For one learner, that might be attending a group session for the first time. For another, it could be travelling independently, developing social confidence, managing anxiety, or preparing for employment.
You'll deliver practical life skills, emotional wellbeing support and personalised learning activities that help young people build confidence, independence and a sense of direction. Most importantly, you'll become a consistent adult who believes in them when they may not yet believe in themselves.

Key Responsibilities
- Deliver personalised 1:1 and small-group sessions focused on life skills, confidence building, emotional wellbeing and individual learner goals.
- Support young people to re-engage with learning at a pace that feels achievable and appropriate for them.
- Plan and adapt learning activities around individual interests, strengths, needs and EHCP outcomes.
- Help learners develop independence, resilience, routines and essential life skills.
- Support transitions between home, community-based activities, centre provision, education, training and employment.
- Deliver Proactive Lifeskills (PALS) and, where appropriate, Functional Skills learning.
- Build positive and trusting relationships with learners who may initially be reluctant to engage.
- Apply trauma-informed and relationship-based approaches to support wellbeing and behaviour.
- Work collaboratively with families, carers, tutors, support staff and external professionals.
- Maintain accurate records of attendance, engagement, progress and learner reviews.
- Contribute to learner assessments, reviews and progression planning.
- Develop and adapt resources that support learners with SEND and SEMH needs.

Essential
- A full UK driving licence and access to a vehicle insured for business use.
- Experience supporting young people with SEND, SEMH needs, anxiety, autism, trauma or related support needs.
- Experience within education, youth work, mentoring, coaching, community support, care, employability services or similar environments.
- The ability to build rapport and trust with young people who may find engagement difficult.
- Experience planning and delivering meaningful 1:1 or small-group activities.
- Strong communication skills and the ability to adapt your approach to different learners.
- Good organisational skills, including record keeping and time management.
- Confidence working independently and managing your own diary.
- Basic digital literacy, including Microsoft Office and online systems.
- Patience, resilience and a reflective approach to your work.
- A genuine belief that every young person can make progress when given the right support.
Desirable
- Level 3 Award in Education and Training (AET) or equivalent.
- Experience delivering Functional Skills.
- Experience working with EHCP outcomes and learner targets.
- Experience creating learning resources, lesson plans or schemes of work.
- Mental health, safeguarding or trauma-informed practice training.
- Experience within alternative education provision.
