Speciality:

Adult Social Care – Community Mental Health Team

To work as a social worker within a multi-disciplinary team, delivering a service to individuals who have severe and enduring mental health problems and or with dementia.

This role is a statutory role and is underpinned by the Mental Health Act 1983, Mental Capacity Act 2005, Social Services and Well-being Act 2014 and the Mental Health (Wales) Measure 2010. The Community Mental Health Team is a busy team working with individuals with severe and enduring mental disorder. The successful candidate will undertake the role within the guiding principles of the Care and Treatment Planning framework. The individual will also undertake s.126 enquires and be responsible for safeguarding the rights of those in their care.

To liaise effectively with other mental health service providers, thereby facilitating the seamless delivery of services to individuals.

To promote the social inclusion of people who experience serious mental illness. To work towards a recovery model of care, in order that individuals can lead independent and fulfilled lives.
